摘要
目的探讨震动感觉阈值(VPT)诊断老年2型糖尿病(T2DM)周围神经病变(DPN)的应用价值。方法选取2018年10月至2021年4月昆明医科大学附属延安医院收治的86例T2DM患者作为研究组,另选取同期健康体检者86名作为对照组。比较两组正中神经、尺神经、腓总神经、腓肠神经VPT值;检测研究组VPT与感觉神经传导速度(SCV)异常率;比较研究组中不同临床表现T2DM患者正中神经、尺神经、腓总神经、腓肠神经VPT值和SCV值。结果研究组正中神经、尺神经、腓总神经、腓肠神经VPT值均高于对照组,差异有统计学意义(P<0.05)。研究组正中神经、尺神经、腓总神经、腓肠神经VPT异常率高于SCV异常率,差异有统计学意义(P<0.05)。研究组中有症状患者正中神经、尺神经、腓总神经、腓肠神经VPT值均高于无症状患者,SCV值均低于无症状患者,差异有统计学意义(P<0.05)。结论VPT诊断老年T2DM周围神经病变可提高诊断准确性,可为临床早期诊断T2DM周围神经病变提供依据,临床应用价值高。
Objective To explore the diagnostic value of vibration perception threshold(VPT)in elderly patients with type 2 diabetes mellitus(T2DM)peripheral neuropathy(DPN).Methods 86 patients with T2DM treated in Yan'an Hospital Affiliated to Kunming Medical University from October 2018 to April 2021 were selected as the study group,and another 86 healthy subjects in the same period were selected as the control group.The VPT values of median nerve,ulnar nerve,common peroneal nerve and sural nerve were compared between the two groups;the abnormal rate of VPT and sensory nerve conduction velocit(SCV)in the study group were detected;the VPT and SCV values of median nerve,ulnar nerve,common peroneal nerve and sural nerve were compared in T2DM patients with different clinical manifestations in the study group.Results The VPT values of median nerve,ulnar nerve,common peroneal nerve and sural nerve in the study group were higher than those in the control group,and the differ-ences were statistically significant(P<0.05).The abnormal rates of VPT in the study group in median nerve,ulnar nerve,common peroneal nerve and sural nerve were higher than those of SCV,and the differences were statistically significant(P<0.05).The VPT values of median nerve,ulnar nerve,common peroneal nerve and sural nerve in symptomatic patients were higher than those in asymptomatic patients,the SCV values of those were lower than those in asymptomatic patients,and the differences were statistically significant(P<0.05).Conclusion VPT can improve the diagnostic accuracy of DPN and provide a basis for early diagnosis of elderly patients with type 2 diabetic peripheral neuropathy,and the clinical application val-ue is high.
