摘要
目的基于数字化医疗服务平台,构建糖尿病药学门诊工作站,探讨糖尿病患者的自我管理水平。方法设计开发糖尿病药学门诊工作站,探索并建立糖尿病药学门诊规范化就诊流程,采用糖尿病自我管理行为量表调查91例2型糖尿病患者的自我管理水平。结果构建糖尿病药学门诊的信息化平台,建立药学门诊规范化就诊流程。只有38.46%的2型糖尿病患者血糖得到控制,自我管理水平也不佳(得分为22.87±7.87)。自我管理行为与空腹血糖和糖化血红蛋白间存在弱相关性,且与“特殊饮食”维度的相关性最显著(P<0.05)。结论药学门诊信息化平台的建立实现了药师参与糖尿病全流程管理的可能性,为创新药学服务模式、提高药学服务质量、改善患者血糖控制及自我管理行为提供了新的可行渠道,具有较好的临床推广价值。
AIM Based on the digital medical service platform,a pharmacist-led diabetes clinic was constructed to explore the self-management level of diabetic patients.METHODS A workstation was designed and developed along with a standardized outpatient procedure for diabetes management.The diabetes electronic self-management behavior scale was used to investigate the self-management level of 91 patients with type 2 diabetes.RESULTS The information platform of pharmacist-led diabetes clinic was constructed,and the standardized treatment process of pharmacy clinics was established.Only 38.46%of participants had their blood glucose controlled while the self-management was also poor(score 22.87±7.87).There were weak correlations between self-management behaviors and fasting blood glucose and glycated hemoglobin,with the most significant correlation in the dimension of"special diet"(P<0.05).CONCLUSION The establishment of the diabetes workstation realizes the possibility of pharmacists participating in the whole process of diabetes management,and provides a new feasible channel for innovative pharmacy service models,improving the quality of pharmacy services,and improving patients’blood glucose control and self-management behaviors,which has good clinical promotion value.
