血清PDCD4、Rac1水平联合CT扫描对膀胱癌及其淋巴结转移的诊断价值

The diagnostic value of serum PDCD4 and Rac1 levels combined with CT scan in bladder cancer and its lymph node metastasis

摘要 目的探讨计算机断层扫描技术(CT)联合血清程序性细胞死亡因子4(PDCD4)、Ras相关C3肉毒菌素底物1(Rac1)对膀胱癌及其淋巴结转移的诊断价值。方法选取2018年1月至2021年12月在本院进行手术治疗的115例膀胱癌患者为观察组, 同时选取100例泌尿系良性疾病患者为对照组。采用实时荧光定量PCR(qRT-PCR)法检测血清中PDCD4、Rac1 mRNA表达水平。采用受试者工作特征(ROC)曲线评价血清PDCD4、Rac1对膀胱癌及其淋巴结转移的诊断价值。分析CT扫描联合血清PDCD4、Rac1水平对膀胱癌及其淋巴结转移的诊断效能。结果观察组的血清PDCD4 mRNA低于对照组, 而Rac1 mRNA高于对照组(均P<0.05)。CT联合PDCD4、Rac1检测诊断膀胱癌的准确率为93.48%, 灵敏度为93.04%, 特异度性为94.00%, 高于CT检测或血清PDCD4、Rac1联合检测。膀胱癌患者发生淋巴结转移的血清PDCD4 mRNA低于未发生淋巴结转移患者, 而Rac1 mRNA高于未发生淋巴结转移患者(均P<0.05)。CT联合PDCD4、Rac1检测诊断膀胱癌患者发生淋巴结转移的准确率为95.65%, 灵敏度为89.65%, 特异度为97.67%, 高于CT检测或PDCD4、Rac1联合检测。结论血清PDCD4、Rac1水平联合CT扫描检测可提高诊断膀胱癌及发生淋巴结转移的准确率和特异度, 具有较高的诊断价值。 Objective To investigate the diagnostic value of computed tomography(CT)combined with serum programmed cell death factor 4(PDCD4)and Ras-related C3 botulinum substrate 1(Rac1)in bladder cancer and its lymph node metastasis.Methods From January 2018 to December 2021,115 bladder cancer patients who came to our hospital for surgical treatment were taken as the observation group,and 100 patients with benign diseases of the urinary system were regarded as the control group.The expression levels of PDCD4 and Rac1 mRNA in serum were detected by real-time quantitative PCR(qRT-PCR).Receiver operating characteristic(ROC)curve was applied to evaluate the diagnostic value of serum PDCD4 and Rac1 in bladder cancer and its lymph node metastasis.The diagnostic efficacy of CT scan combined with serum PDCD4 and Rac1 levels in bladder cancer and its lymph node metastasis was analyzed.Results The accuracy rate of CT combined with PDCD4 and Rac1 detection in the diagnosis of bladder cancer was 93.48%,the sensitivity was 93.04%,and the specificity was 94.00%,which was higher than the accuracy/specificity of CT detection or serum PDCD4 combined Rac1;the accuracy rate of CT combined with PDCD4 and Rac1 detection in diagnosing lymph node metastasis of bladder cancer was 95.65%,the sensitivity was 89.65%,and the specificity was 97.67%,which was higher than CT or PDCD4 and Rac1 combined detection.Conclusions Serum PDCD4 and Rac1 levels combined with CT scan can improve the accuracy and specificity of bladder cancer and lymph node metastasis,and have high diagnostic value.
