摘要
目的探讨MRI诊断膝关节损伤的价值。方法选取2020年1月至2022年10月于医院就诊的100例疑似膝关节损伤患者作为研究对象,均行MRI和关节镜检查。以关节镜检查结果为金标准,分析MRI诊断膝关节损伤类型(包括关节腔积液、半月板损伤、韧带损伤)与关节镜检查结果的符合率,MRI诊断膝关节损伤的灵敏度、特异度、准确度及与关节镜检查结果的一致性。结果关节镜检查结果显示,100例疑似膝关节损伤患者中,86例存在膝关节损伤,包括关节腔积液20例、半月板损伤36例、韧带损伤30例。MRI诊断关节腔积液、半月板损伤、韧带损伤与关节镜检查结果的符合率分别为95.00%、94.44%、90.00%,总符合率为93.02%。MRI诊断膝关节损伤的灵敏度、特异度、准确度分别为90.70%、85.71%、90.00%,与关节镜检查结果的一致性较高(Kappa=0.648,P<0.05)。结论MRI诊断膝关节损伤类型与关节镜检查结果的符合率均较高,且诊断灵敏度、特异度、准确度较高,与关节镜检查结果的一致性较高。
Objective To explore the value of MRI in diagnosing knee joint injury.Methods With the selection of 100 patients with suspected knee joint injury who visited hospitals from January 2020 to October 2022 as the study subjects,all patients underwent MRI and arthroscopy examinations.With arthroscopic examination results as the gold standard,the consistency between MRI diagnosis of knee joint injury types(included joint cavity effusion,meniscus injury,ligament injury)and arthroscopic examination results was analyzed,as well as the sensitivity,specificity,accuracy,and consistency with arthroscopic examination results in diagnosing knee joint injury.Results The results of arthroscopic examination showed that 86 out of 100 suspected patients had knee joint injuries,included 20 cases of joint cavity effusion,36 cases of meniscus injury,and 30 cases of ligament injury.The consistency rates between MRI diagnosis of joint effusion,meniscus injury,ligament injury and arthroscopic examination results were 95.00%,94.44%,and 90.00%,respectively,with a total consistency rate of 93.02%.The sensitivity,specificity,and accuracy of MRI in diagnosing knee joint injury were 90.70%,85.71%,and 90.00%,respectively,with high consistency with the results of arthroscopy(Kappa=0.648,P<0.05).Conclusion The consistency between MRI diagnosis of knee joint injury types and arthroscopic examination results is high,with high diagnostic sensitivity,specificity,and accuracy,and high consistency with arthroscopic examination results.
