粪便SDC2基因甲基化联合CEA检测在早期结直肠癌鉴别中的应用价值

摘要 目的:探究粪便硫酸类肝素蛋白多糖2(syndecan-2,SDC2)基因甲基化联合癌胚抗原(carcinoembryonic antigen,CEA)检测在早期结直肠癌鉴别中的应用价值。方法:选择2020年8月—2022年10月于河南科技大学第一附属医院胃肠外科接受诊疗的86例疑似早期结直肠癌患者为研究组,选择同期于本院接受治疗的32例良性结直肠病变患者为对照组,对其分别进行粪便SDC2基因甲基化检测及CEA水平检测,以病理学结果为金标准,分别计算两种检测方式及联合检测对早期结直肠癌的诊断效能。结果:研究组粪便SDC2基因甲基化阳性占比及CEA水平均高于对照组,差异有统计学意义(P<0.05)。粪便SDC2基因甲基化联合血清CEA检测早期结直肠癌敏感度、准确度高于血清CEA检测,差异有统计学意义(P<0.05);但粪便SDC2基因甲基化联合血清CEA检测早期结直肠癌敏感度、特异度和准确度与粪便SDC2基因甲基化及粪便SDC2基因甲基化联合血清CEA检测早期结直肠癌特异度与血清CEA检测比较,差异无统计学意义(P>0.05)。结论:粪便SDC2基因甲基化检测及CEA水平对早期结直肠癌均具有一定的诊断效能,而联合检测有助于提高诊断敏感度。 Objective:To explore the application value of fecal syndecan-2(SDC2)gene methylation combined with carcinoembryonic antigen(CEA)detection in the differential diagnosis of early colorectal cancer.Method:A total of 86 patients with suspected early colorectal cancer who treated in the Department of Gastrointestinal Surgery,the First Affiliated Hospital of Henan University of Science and Technology from August 2020 to October 2022 were selected as the study group,and 32 patients with benign colorectal lesions who treated in our hospital during the same period were selected as the control group.Fecal SDC2 gene methylation detection and CEA level detection were performed respectively,the pathological results were used as the gold standard to calculate the diagnostic efficacy of two detection methods and combined detection for early colorectal cancer.Result:The proportion of positive fecal SDC2 gene methylation and CEA levels in the study group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).The sensitivity and accuracy of fecal SDC2 gene methylation combined with serum CEA in the detection of early colorectal cancer were higher than those of serum CEA detection,and the differences were statistically significant(P<0.05);however,there were no statistically significant differences in the sensitivity,specificity and accuracy of fecal SDC2 gene methylation combined with serum CEA in the detection of early colorectal cancer compared with those of fecal SDC2 gene testing,the specificity of fecal SDC2 gene methylation combined with serum CEA compared with that of serum CEA in the detection of early colorectal cancer(P>0.05).Conclusion:Fecal SDC2 gene methylation detection and CEA levels have certain diagnostic efficacy for early colorectal cancer,and combined detection is helpful to improve the diagnostic sensitivity.
