摘要
目的:基于ADOPT模式,深入了解脑卒中吞咽障碍患者的自我管理需求,为脑卒中吞咽障碍患者的护理提供参考。方法:采用目的抽样法,选取2022年11月-2023年3月湖南省某三级甲等医院脑卒中吞咽障碍患者作为研究对象。运用质性研究的现象学方法对脑卒中吞咽障碍患者进行深度访谈,运用Colaizzi七步法分析资料,并提炼主题。结果:该研究最终纳入25例患者,共提炼出5个主题,包括心理负担重,缺乏康复信心(态度);吞咽障碍的评估与自我管理效果欠佳(定义);患者自我管理的支持工作不足(开放思维);患者自我管理的主动性差(计划);患者自我管理的参与度低(实施)。结论:脑卒中吞咽障碍患者存在多方面的自我管理需求,护理人员应全面、深入掌握患者的各种需求,调动患者参与自我管理的主动性,提升患者自我管理能力,促进患者功能恢复。
Objective:To deeply understand the self-management needs of stroke patients with dysphagia based on ADOPT model,and to provide reference for nursing care of stroke patients with dysphagia.Methods:From November 2022 to March 2023,stroke patients with dysphagia in a tertiary A-level hospital in Hunan Province were selected as the study subjects by means of objective sampling.The phenomenological method of qualitative research was used to conduct in-depth interviews on stroke patients with dysphagia.Colaizzi seven-step method was used to analyze the data and extract the themes.Results:A total of 25 patients were finally included in this study and 5ive themes were extracted,including heavy psychological burden and lack of recovery confidence(attitude);poor assessment and self-management of swallowing disorders(definition);inadequate support for self-management of patients(open mind);poor self-management initiative(planning);low participation in self-management(implementation).Conclusion:Stroke patients with dysphagia have various needs for self-management,and nursing staff should comprehensively and deeply grasp the various needs of patients,mobilize the initiative of patients to participate in self-management,improve the self-management ability of patients,and promote the functional recovery of patients.
