摘要
目的探讨腓肠神经小隐静脉营养血管皮瓣修复联合负压封闭引流(VSD)在足踝软组织缺损患者中的应用效果。方法选取2019年1月至2022年1月收治的108例足踝软组织缺损患者为研究对象,随机将其分为对照组和观察组,各54例。对照组采用腓肠神经小隐静脉营养血管皮瓣修复治疗,观察组在对照组基础上联合VSD治疗。比较两组的治疗效果。结果观察组的感觉功能分级优于对照组(P<0.05)。治疗后,观察组的C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平低于对照组(P<0.05)。治疗后,观察组的血流灌注指数、经皮氧分压(TcPO_(2))、血管内皮生长因子(VEGF)水平高于对照组(P<0.05)。观察组的足功能恢复优良率高于对照组(P<0.05)。结论腓肠神经小隐静脉营养血管皮瓣修复联合VSD治疗足踝软组织缺损可进一步改善患者皮瓣感觉功能,减轻炎症,加快创面愈合速度,提高足踝功能恢复效果。
Objective To investigate the application effect of sural nerve small saphenous vein nutritional vascular flap repair combined with vacuum sealing drainage(VSD)in patients with foot and ankle soft tissue defect.Methods A total of 108 patients with foot and ankle soft tissue defect admitted from January 2019 to January 2022 were selected as the study objects and randomly divided into control group and observation group,with 54 cases in each group.The control group was treated with sural nerve small saphenous vein nutritional vascular flap repair,and the observation group was treated with VSD on the basis of the control group.The therapeutic effects of the two groups were compared.Results The sensory function grading of the observation group was better than that of the control group(P<0.05).After treatment,the levels of C-reactive protein(CRP),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in the observation group were lower than those in the control group(P<0.05).After treatment,the blood perfusion index,transcutaneous oxygen pressure(TcPO_(2))and vascular endothelial growth factor(VEGF)level in the observation group were higher than those in the control group(P<0.05).The excellent and good rate of foot function recovery in the observation group was higher than that in the control group(P<0.05).Conclusion The sural nerve small saphenous vein nutritional vascular flap repair combined with VSD in the treatment of foot and ankle soft tissue defect can further improve the sensory function of the flap,reduce inflammation,accelerate wound healing,and improve the recovery of foot and ankle function.
