摘要
目的观察环硅酸锆钠对尿毒症血液透析患者营养状态的影响。方法本研究采用前瞻性巢式病例对照设计。从2022年1月至2022年6月在我院血液透析的尿毒症患者中纳入21例作为治疗组,口服环硅酸锆钠控制血钾;同时按照年龄、性别、身体质量指数、基线血钾及透析龄进行倾向性评分匹配,匹配21例尿毒症患者作为对照组,仅通过饮食限制控制血钾。比较两组患者的血钾、食物频率、血红蛋白、血清白蛋白、身体质量指数、肱三头肌皮褶厚度和营养不良-炎症评分等指标。结果与对照组比较,治疗组的血钾、营养不良-炎症评分降低(P均<0.05),食物多样性、血红蛋白、血清白蛋白、身体质量指数、肱三头肌皮褶厚度、生活质量调查表生理评分均明显升高(P均<0.05)。结论环硅酸锆钠可用以控制尿毒症血液透析患者的高钾血症,使患者的饮食限制放宽,从而改善患者的营养状态。
Objective To observe the effect of sodium zirconium cyclosilicate on the nutritional status of hemodialysis patients with uremia.Methods A prospective nested case-control design was used in this study.From January 2022 to June 2022,21 uremic patients undergoing hemodialysis in our hospital were included as the treatment group,in which sodium zirconium cyclosilicate was orally administered to control blood potassium.At the same time,after propensity score matching was performed based on age,gender,body mass index,baseline blood potassium,and dialysis age,21 uremic patients were paired as the control group,in which blood potassium was controlled only through dietary restriction method.The two groups were compared in indicators of blood potassium,food frequency,hemoglobin,serum albumin,body mass index,triceps skinfold thickness,and malnutrition-inflammation score,etc.Results Compared with the control group,the treatment group showed a decrease in blood potassium and malnutrition-inflammation score(P<0.05),and a significant increase in food diversity,hemoglobin,serum albumin,body mass index,triceps skinfold thickness,and physiological scores of the quality of life questionnaire(P<0.05).Conclusion In the hemodialysis patients with uremia,sodium zirconium cyclosilicate could be used to control their hyperkalemia,relax their dietary restrictions,and improve their nutritional status.
