摘要
目的 探讨角蛋白19(KRT19)和微小RNA(miRNA)-623在甲状腺癌组织中的表达及与患者预后的关系。方法 收集95例甲状腺癌患者的甲状腺癌组织和癌旁组织,采用定量逆转录聚合酶链反应(qRT-PCR)检测甲状腺癌组织和癌旁组织中KRT19 mRNA、miRNA-623相对表达量,比较不同临床特征甲状腺癌患者甲状腺癌组织中KRT19、miRNA-623表达情况。对所有患者进行3年随访,比较不同KRT19、miRNA-623表达情况甲状腺癌患者的无病生存情况。根据随访结果将患者分为预后良好组和预后不良组,比较两组患者的KRT19、miRNA-623相对表达量。绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),分析KRT19、miRNA-623单独及联合检测对甲状腺癌患者预后的预测价值。采用Cox回归模型分析甲状腺癌患者预后的影响因素,采用Pearson相关分析法分析甲状腺癌组织中KRT19与miRNA-623表达的相关性。结果 甲状腺癌组织中KRT19 mRNA相对表达量明显高于癌旁组织,miRNA-623相对表达量明显低于癌旁组织,差异均有统计学意义(P﹤0.01)。肿瘤直径≥1 cm、有淋巴结转移的甲状腺癌患者甲状腺癌组织中KRT19高表达率分别高于肿瘤直径﹤1 cm、无淋巴结转移的患者(P﹤0.05);肿瘤直径≥1 cm、有淋巴结转移的甲状腺癌患者甲状腺癌组织中miRNA-623高表达率分别低于肿瘤直径﹤1 cm、无淋巴结转移的患者(P﹤0.05)。随访3年,KRT19高表达患者的无病生存率为59.57%,低于KRT19低表达患者的85.42%(P﹤0.05);miRNA-623高表达患者的无病生存率为82.98%,高于miRNA-623低表达患者的62.50%(P﹤0.05)。预后不良组患者KRT19 mRNA相对表达量明显高于预后良好组,miRNA-623相对表达量明显低于预后良好组,差异均有统计学意义(P﹤0.01)。ROC曲线显示,KRT19、miRNA-623联合检测预测甲状腺癌患者预后的AUC、灵敏度、特异度、准确度均高于二者单独检测。有淋巴结转移、KRT19高表达、miRNA-6
Objective To explore the expression of keratin 19(KRT19) and microRNA(miRNA)-623 in thyroid cancer tissues and their relationship with prognosis of patients.Method Thyroid cancer tissues and paracancerous tissues of95 patients with thyroid cancer were collected.Quantitative reverse transcription-polymerase chain reaction(qRT-PCR)was performed to detect the relative expression levels of KRT19 m RNA and miRNA-623 in thyroid cancer tissues and paracancerous tissues,and the expression of KRT19 and miRNA-623 were also compared in thyroid cancer patients with different clinical characteristics.All patients were followed up for 3 years to compare the disease-free survival of thyroid cancer patients with different expression levels of KRT19 and miRNA-623.According to the follow-up results,the patients were divided into good prognosis group and poor prognosis group,and the relative expression levels of KRT19mRNA and miRNA-623 were compared between the two groups.Receiver operating characteristic(ROC) curve was plotted,area under the curve(AUC) was calculated,and the prognostic value of KRT19 and miRNA-623 alone and combined detection in thyroid cancer patients was analyzed.Cox regression analysis was used to analyze the factors affecting the prognosis of patients with thyroid cancer,and Pearson correlation analysis was performed to evaluate the correlation between KRT19 and miRNA-623 expression in thyroid cancer tissues.Result The relative expression level of KRT19mRNA in thyroid cancer tissues was significantly higher than that in paracancerous tissues,and the relative expression level of miRNA-623 was significantly lower than that in paracancerous tissues(P<0.01).The high expression rate of KRT19 in thyroid cancer tissues of thyroid cancer patients with tumor diameter≥1 cm and lymph node metastasis were higher than those of patients with tumor diameter<1 cm and no lymph node metastasis(P<0.05).The high expression rate of miRNA-623 in thyroid cancer tissues of thyroid cancer patients with tumor diameter≥1 cm and lymp
