摘要
目的探究分化型甲状腺癌(DTC)患者术前血清微小RNA-599(microRNA-599)及缺氧诱导因子-2α(HIF-2α)水平与预后的关系。方法选取2017年1月至2017年6月我院收治的98例DTC患者(记为DTC组)及良性甲状腺结节患者98例(良性组)为研究对象,比较两组术前血清microRNA-599及HIF-2α水平,并分析DTC患者血清microRNA-599及HIF-2α表达水平与临床病理特征及预后的关系。结果与良性组相比较,DTC组血清microRNA-599水平明显降低、HIF-2α阳性表达率明显升高(P<0.001);DTC复发、分化程度低患者血清microRNA-599水平相较于无复发、分化程度高者明显降低(P<0.05),而DTC复发、分化程度低患者HIF-2α阳性表达率相较于无复发、分化程度高者明显升高(P<0.05);并且DTC患者预后复发与血清microRNA-599水平呈明显的负相关(r=-0.506,P<0.001),而与HIF-2α阳性表达呈明显正相关(r=0.530,P<0.001),此外血清microRNA-599低表达组/HIF-2α阳性表达组中位无进展生存期较高表达组/阴性表达组明显缩短(P<0.05)。结论DTC患者预后复发、无进展生存期与术前血清microRNA-599、HIF-2α表达显著相关,术前严密监测DTC血清microRNA-599、HIF-2α表达情况有利于早期防治方案的制定。
Objective To explore the relationship between preoperative serum microRNA-599(microRNA-599),hypoxia-inducible factor-2α(HIF-2α)levels and prognosis in patients with differentiated thyroid cancer(DTC).Methods 98 DTC patients(the DTC group)and 98 patients with benign thyroid nodules(the benign group)who were admitted to the hospital from January,2017 to June,2017 were enrolled for the study.The preoperative levels of serum microRNA-599 and HIF-2αwere compared between the two groups.The relationship between expression levels of serum microRNA-599 and HIF-2αand clinicopathological features and prognosis in DTC patients was analyzed.Results Compared with the benign group,the level of serum microRNA-599 was significantly lower,while the positive rate of HIF-2αwas significantly higher in the DTC group(P<0.001).The level of serum microRNA-599 in patients with DTC recurrence and low differentiation degree was significantly lower than those with non-recurrence and high differentiation degree(P<0.05),while the positive expression rate of HIF-2αin patients with DTC recurrence and low differentiation degree was significantly higher than those with non-recurrence and high differentiation degree(P<0.05).The prognosis recurrence of DTC patients was significantly negatively correlated with serum microRNA-599 level(r=-0.506,P<0.001),while significantly positively correlated with positive expression of HIF-2α(r=0.530,P<0.001).In addition,median progression-free survival in the serum microRNA-599 low expression group/HIF-2αpositive expression group was significantly shorter than those in the high expression group/negative expression group(P<0.05).Conclusion The prognosis recurrence and progression-free survival of DTC patients are significantly correlated with preoperative serum microRNA-599 and HIF-2αexpressions.A close monitoring on preoperative serum microRNA-599 and HIF-2αexpressions in DTC patients could be beneficial to the development of the early clinical control plans.
