摘要
目的:分析动态心电图检查对缺血性心肌病伴高血压患者的诊断价值。方法:选取2020年1月—2022年12月遵义市妇幼保健院收治的90例缺血性心肌病伴高血压患者作为研究对象,所有患者行常规心电图、动态心电图、冠脉造影检查,以冠脉造影结果为金标准,比较两种检查方法的诊断效能。结果:两组灵敏度比较,差异无统计学意义(P=0.365);动态心电图诊断缺血性心肌病合并高血压的特异性高于常规心电图,差异有统计学意义(P=0.025);动态心电图对心肌缺血部位的检出率高于常规心电图,差异有统计学意义(P=0.016)。缺血性心肌病伴高血压患者发生QRS时限异常、左室高压异常、心律失常的概率均高于扩张性心肌病患者,差异有统计学意义(P<0.05)。缺血性心肌病伴高血压患者最大、最小及矫正Tp-e间期均短于扩张型心肌病伴高血压患者,差异有统计学意义(P<0.001)。结论:动态心电图诊断缺血性心肌病合并高血压的灵敏度与常规心电图相当,特异性较高,且检出心肌缺血部位较准确。
Objective:To analyze the diagnostic value of Holter monitoring on patients with ischemic cardiomyopathy and hyper⁃tension.Methods:90 patients with ischemic cardiomyopathy and hypertension admitted to Zunyi Maternal and Child Health Hospi⁃tal from January 2020 to December 2022 were selected as the study subjects,and all the patients were examined by conventional electrocardiogram,Holter monitor and coronary angiography,and the results of coronary angiography were used as the gold stan⁃dard to compare the diagnostic efficacy of the another two examination methods.Results:Comparing the sensitivity of the two groups,the difference was not statistically significant(P=0.365);the specificity of holter in diagnosing ischemic cardiomyopathy and hypertension was higher than that of conventional electrocardiography,and the difference was statistically significant(P=0.025);and the detection rate of myocardial ischemic sites with Holter monitor was higher than that of conventional electrocardiogra⁃phy,and the difference was statistically significant(P=0.016).Patients with ischemic cardiomyopathy and hypertension were more likely to have abnormal QRS time limits,abnormal left ventricular hypertension,and arrhythmias than patients with dilated cardio⁃myopathy,and the difference was statistically significant(P<0.05).The maximum,minimum and corrected Tp-e intervals were shorter in patients with ischemic cardiomyopathy with hypertension than in patients with dilated cardiomyopathy with hypertension,and the difference was statistically significant(P<0.001).Conclusion:Holter monitor in the diagnosis of ischemic cardiomyopathy and hypertension contributes to higher specificity in detecting myocardial ischemia sites compared to normal electrocardiogram.
