原发性高血压并室性心律失常患者血压昼夜节律和心率震荡的相关性 被引量:17

Correlation between blood pressure circadian rhythm and heart rate oscillation in patients with primary hypertension and ventricular arrhythmia

摘要 目的研究并探讨原发性高血压(高血压)合并室性心律失常患者血压昼夜节律与心率震荡的相关性。方法选择2015年1月至2017年7月在韶关市第一人民医院接受住院治疗的120例高血压合并室性心律失常患者设置为疾病组,另选取同期100例健康体检者设置为对照组,采用12导联24 h动态心电图、24 h动态血压检测仪对其进行双动态监测,比较其心率震荡指标、血压昼夜节律指标。根据Lown氏分级法将疾病组分为良性室性心律失常亚组、不良室性心律失常亚组,比较组间心率震荡指标。根据动态血压昼夜节律将疾病组分为杓型亚组、非杓型亚组,比较组间心率震荡指标。采用Pearson相关分析法,分析心率震荡、血压昼夜节律与高血压合并室性心律失常的相关性。结果疾病组的窦性心律震荡初始值(turbulence onset,TO)、动态心率震荡(turbulence dynamics,TD)均高于对照组,其震荡斜率(turbulence slope,TS)低于对照组,差异有统计学意义(P<0.05)。不良室性心律失常亚组的TO、TD均高于良性室性心律失常亚组,其TS低于良性室性心律失常亚组,差异有统计学意义(P<0.05)。非杓型亚组的TO高于杓型亚组,其TS、TD低于杓型亚组,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示,心率震荡指标中的TO(r=0.725,P=0.001)、TD(r=0.704,P=0.003)、血压昼夜节律指标与高血压合并室性心律失常呈正相关,TS(r=-0.713,P=0.002)与室性心律失常呈负相关。结论心率震荡、血压昼夜节律与高血压合并室性心律失常的发生密切相关,临床上可将窦性心率震荡、TD、血压昼夜节律作为判断高血压并发室性心律失常患者自主神经功能损害情况的指标。 Objectives To study and explore the correlation between blood pressure circadian rhythm and heart rate oscillation in patients with primary hypertension combined with ventricular arrhythmia.Methods From January 2015 to July 2017 in the First People′s Hospital of Shaoguan City,120 patients with primary hypertension combined with ventricular arrhythmia were set as disease group.At the same period,100 cases of healthy physical examination were set as control group.The 12 lead 24 hours dynamic electrocardiogram,24-hour ambulatory blood pressure detector were used to monitor the two groups.Sinus heart rate volatility index,index of blood pressure circadian rhythm were compared between the two groups.According to the Lown classification method,patients in disease group were divided into benign ventricular arrhythmia subgroup and adverse ventricular arrhythmia subgroup.According to circadian rhythm of blood pressure,patients in disease group were divided into arytenoid subgroup and the non-arytenoid subgroup.Sinus rhythm index and circadian rhythm index of blood pressure were compared between the two groups.Using Pearson correlation coefficient analysis to analyzed the correlations between sinus rhythm fluctuation,blood pressure circadian rhythm and primary hypertension combined with ventricular arrhythmia.Results Turbulence onset(TO)and turbulence dynamics(TD)of disease group were significantly higher than thaose of control group(P<0.05),and turbulence slope(TS)was significantly lower than that of control group(P<0.05).TO and TD in adverse ventricular arrhythmia subgroup were significantly higher than those in benign ventricular arrhythmia subgroup(P<0.05),and TS was obviously lower than that in benign ventricular arrhythmia subgroup(P<0.05).TO in non-arytenoid subgroup was significantly higher than that in arytenoid subgroup(P<0.05),and TS,TD were obviously lower than those in arytenoid subgroup(P<0.05).Pearson correlation analysis indicated that TO(r=0.725,P=0.001),TD(r=0.704,P=0.003),circadian rhythm index of bloo
