摘要
目的探讨超声引导下短轴平面外结合长轴平面内法在小儿股静脉穿刺置管中的应用效果。方法40例择期行心脏开胸房室缺修补术和微创介入封堵术患儿,按随机数字表法分为对照组与研究组,每组20例。对照组使用短轴平面外法行股静脉穿刺置管,研究组使用短轴平面外结合长轴平面内法行股静脉穿刺置管。比较两组首次穿刺成功率、穿刺置管总时长及并发症发生情况。结果两组患儿均完成超声引导下股静脉穿刺置管。研究组首次穿刺成功率85%高于对照组的55%,差异有统计学意义(P<0.05)。研究组穿刺置管总时长(312±17)s短于对照组的(483±27)s,差异有统计学意义(P<0.05)。研究组血肿发生率5%低于对照组的20%,差异有统计学意义(P<0.05)。两组均未发生意外股动脉穿刺情况。结论超声引导短轴平面外结合长轴平面内法在小儿股静脉穿刺置管中首次穿刺成功率高,穿刺时间缩短,血肿发生率低,值得推广。
Objective To explore the practical effect of ultrasound-guided short axis out-of-plane combined with long axis in-plane method for femoral vein catheterization in children.Methods 40 children undergoing elective cardiac thoracotomy,atrioventricular defect repair and minimally invasive interventional occlusion were divided into a control group and a study group according to random number table method,with 20 cases in each group.The control group received short axis out-of-plane method for femoral vein catheterization,and the study group received short axis out-of-plane combined with long axis in-plane method for femoral vein catheterization.The success rates of the first puncture,the total duration of puncture and catheterization,and the occurrence of complications were compared between the two groups.Results Both groups of children completed ultrasound-guided femoral vein catheterization.The success rate of the first puncture in the study group was 85%,which was higher than that of 55%in the control group,and the difference was statisticallly significant(P<0.05).The total duration of puncture and catheterization in the study group was(312±17)s,which was significantly longer than that of(483±27)s in the control group,and the difference was statisticallly significant(P<0.05).The incidence of hematoma of 5%in the study group was lower than that of 20%in the control group,and the difference was statistically significant(P<0.05).No accidental femoral artery puncture occurred in the two groups(P>0.05).Conclusion Ultrasound-guided short axis out-of-plane combined with long axis in-plane method in femoral vein catheterization in children has high success rates of the first puncture and catheterization,shortened puncture time,and low incidence of hematoma,which is worth popularizing.
