摘要
目的探讨手术决策参与及医患信任对骨科患者术前焦虑的影响路径。方法采用一般资料调查表、阿姆斯特丹术前焦虑和信息量表、患者对医疗决策参与的满意度调查问卷、维克森林医师信任量表对231例骨科手术患者进行问卷调查。结果骨科手术患者术前焦虑发生率为37.66%,手术决策参与满意度得分为78.96(70.42,81.46)分、医患信任得分为34.00(27.00,40.00)分;结构方程模型结果显示,手术决策参与满意度(β=-0.165,P=0.017)及医患信任(β=-0.238,P<0.001)对患者术前焦虑均有直接预测作用,手术决策参与满意度还能够通过影响医患信任间接作用于患者术前焦虑[间接效应95%CI(-0.221,-0.028)],间接效应占比为36.54%。结论手术决策参与满意度及医患信任共同影响骨科患者的术前焦虑,建议采取多种措施提高患者的手术决策参与满意度,改善医患信任,以降低患者术前焦虑。
Objective To explore the influencing pathway of surgical decision-making participation and doctor-patient trust on preop-erative anxiety in orthopedic patients.Methods A total of 231 orthopedic surgery patients were investigated with the general information questionnaire,the Amsterdam Preoperative Anxiety and Information Scale,the Scale of Patient Satisfaction with Medical Decision-Making Participation and the Wake Forest Physician Trust Scale.Results The incidence of preoperative anxiety in patients undergoing orthopedic surgery was 37.66%.The score of satisfaction with surgical decision-making participation was 78.96(70.42,81.46),and the score of doctor-patient trust was 34.00(27.00,40.00).The structural equation model showed that satisfaction with surgical decision-making participation(β=-0.165,P=0.017)and doctor-patient trust(β=-0.238,P<0.001)had a direct predictive effect on preoperative anxiety in patients.Satisfaction with surgical decision-making participation could also indirectly affect the preoperative anxiety in patients by influencing doctor-patient trust[indirect effect 95%CI(-0.221,-0.028)],with an indirect effect proportion of 36.54%.Conclusion The satisfaction of surgi-cal decision-making participation and doctor-patient trust affect the preoperative anxiety of orthopedic patients.It is recommended to take a variety of measures to improve the satisfaction of surgical decision-making participation and doctor-patient trust,so as to reduce the preopera-tive anxiety of patients.
