摘要
目的:研究改良呼吸道湿化法在预防危重症患者呼吸机相关性肺炎(VAP)中的应用效果。方法:选择本院2021年6月至2023年6月间收治的需呼吸机辅助呼吸的危重症患者80例,采用随机数字表法分为对照组及观察组,每组各40例。对照组给予普通气道湿化法,观察组给予改良呼吸道湿化法,比较两组患者痰液黏稠度、痰痂形成率、刺激性咳嗽次数、机械通气时间、肺部湿啰音消失时间、舒适度(GCQ)评分及VAP发生率。结果:观察组痰液黏稠度、痰痂形成率及刺激性咳嗽次数均低于对照组(P<0.05)。观察组机械通气时间及肺部湿啰音消失时间均短于对照组(P<0.05)。观察组GCQ评分明显高于对照组,且观察组VAP发生率为2.50%,对照组为15%,观察组低于对照组(P<0.05)。结论:改良呼吸道湿化法可以有效减轻危重症机械通气患者痰液粘稠度,避免痰痂形成,降低刺激性咳嗽次数,还可缩短机械通气及肺部湿啰音消失时间,能够提高患者舒适度并降低VAP发生率。
Objective:To study the effect of modified respiratory humidification in the prevention of ventilatorassociated pneumonia(VAP)in critically ill patients.Methods:A total of 80 critically ill patients with respiratory muscle assisted respiration admitted to our hospital from June 2021 to June 2023 were divided into control group and observation group by random number table method,with 40 cases in each group.The control group was given the common airway humidification method,and the observation group was given the improved airway humidification method.The sputum viscosity,the rate of phlegm scab formation,the frequency of irritating cough,the time of mechanical ventilation,the time of lung moist rale disappearance,the score of comfort(GCQ)and the incidence of VAP were compared between the two groups.Results:The consistency of sputum,the rate of phlegm scab formation and the frequency of irritating cough in observation group were lower than those in control group(P<0.05).The mechanical ventilation time and the disappearance time of lung moist rales in the observation group were shorter than those in the control group(P<0.05).The GCQ score of the observation group was significantly higher than that of the control group,and the incidence of VAP was 2.50%in the observation group and 15%in the control group,and the observation group was lower than the control group(P<0.05).Conclusion:Improved respiratory humidification can effectively reduce the phlegm viscosity,avoid the formation of phlegm scab,reduce the frequency of irritating cough,shorten the time of mechanical ventilation and lung moist rale disappearance,improve the comfort of patients and reduce the incidence of VAP.
