摘要
目的探究益生菌联合新癀片对牙周病患者牙周状态、口腔致病菌群及龈沟液中骨代谢指标水平的影响。方法选取168例慢性牙周炎患者,随机分为3组,各56例。对照A组予以益生菌治疗,对照B组予以新癀片治疗,实验组予以益生菌联合新癀片治疗。比较3组临床疗效、不良反应及治疗前、疗程结束后、治疗后3个月牙龈出血指数(BI)、牙周探诊深度(PD)、临床附着丧失(AL)、碱性磷酸酶(ALP)、骨钙素(BGP)、骨保护素(OPG)、白细胞介素(IL)-1β、6及肿瘤坏死因子(TNF)-α水平及牙龈仆啉单胞菌、伴放线放线杆菌检出率。结果实验组治疗总有效率较对照A、B组高(P<0.05),3组不良反应发生率比较,差异无统计学意义(P>0.05);实验组疗程结束后、治疗后3个月BI、PD、AL、IL-1β、IL-6、TNF-α水平及牙龈仆啉单胞菌和伴放线放线杆菌检出率均较对照A、B组低,ALP、BGP、OPG水平均较对照A、B组高(P<0.05);对照A、B组BI、PD、AL、ALP、BGP、OPG、IL-1β、IL-6、TNF-α水平及牙龈仆啉单胞菌和伴放线放线杆菌检出率比较,差异无统计学意义(P>0.05)。结论益生菌联合新癀片治疗牙周病,可显著改善患者牙周状态及骨代谢情况,减少口腔致病菌群,减轻炎症反应,效果更为显著。
Objective To investigate the effect of probiotics combined with Xinhuang tablets on oral pathogenic flora and the level of bone metabolic indexes in gingival sulcus fluid of patients with periodontal disease.Methods A total of 168 patients with chronic periodontitis were selected and randomly divided into 3 groups with 56 cases in each group.The control group A was treated with probiotics,the control group B was treated with Xinhuang tablets,and the experimental group was given probiotics combined with Xinhuang tablets.The clinical efficacy,adverse reactions,gingival crevicular bleeding index(BI),periodontal pocket probe depth(PD),clinical attachment loss(AL),alkaline phosphatase(ALP),osteocalcin(BGP),osteoprotegerin(OPG),interleukin(IL)-1β,6 and tumor necrosis factor(TNF)-αlevels and porphyromonas gingivalis actinbacillus actinom gcetemcomitans detection rate before and after treatment,and 3 months after treatment were compared among the three groups.Results The total effective rate of treatment was higher in the experimental group than that in control groups A and B(P<0.05),and there was no statistically significant difference in the incidence of adverse reactions among the three groups(P>0.05).BI,PD,AL,IL-1β,IL-6,TNF-αlevels and the detection rates of Porphyromonas gingivalis and associated actinobacillus actinom gcetemcomitans at the end of the treatment course and 3 months after treatment were lower in the experimental group than in control groups A and B and the levels of ALP,BGP and OPG were higher than those of control groups A and B(P<0.05).Conclusions Probiotics combined with Xinhuang tablet in the treatment of periodontal disease can significantly improve the periodontal status and bone metabolism of patients,reduce oral pathogenic flora and inflammatory reaction.
