摘要
目的:观察穴位贴敷、艾灸联合综合康复疗法治疗脑卒中后上肢功能障碍的临床疗效。方法:选取80例脑卒中后上肢功能障碍患者,按随机数字表法分成对照组和试验组各40例。对照组给予综合康复疗法治疗,试验组在对照组基础上给予穴位贴敷联合艾灸治疗,2组均治疗4周。比较2组临床疗效及治疗前后改良Ashworth量表(MAS)、Fugl-Meyer上肢运动功能评定量表(FMA-UE)、改良Barthel指数(MBI)评分。结果:治疗后,试验组总有效率92.50%,高于对照组75.00%(P<0.05)。2组腕屈肌群、肘屈肌群、肩内旋肌群MAS评分均较治疗前降低(P<0.05),试验组腕屈肌群、肘屈肌群、肩内旋肌群MAS评分均低于对照组(P<0.05)。2组FMA-UE、MBI评分均较治疗前升高(P<0.05),试验组FMA-UE、MBI评分均高于对照组(P<0.05)。结论:穴位贴敷、艾灸联合综合康复疗法治疗脑卒中后上肢功能障碍临床疗效较好,能降低患侧上肢各肌群的肌张力,促进上肢功能恢复,提高自理能力。
Objective:To observe the clinical effect of acupoint application and moxibustion combined with comprehensive rehabilitation therapy for upper limb dysfunction after stroke.Methods:A total of 80 cases of patients with upper limb dysfunction after stroke were selected and divided into the control group and the trial group according to the random number table method,with 40 cases in each group.The control group was treated with comprehensive rehabilitation therapy,and the trial group was additionally treated with acupoint application and moxibustion based on the treatment of the control group.Both groups were treated for 4 weeks.The scores of Modified Ashworth Scale(MAS),Fugl-Meyer Assessment of Upper Extremity Motor Function(FMA-UE),and Modified Barthel Index(MBI)before and after treatment,and the clinical effects were compared between the two groups.Results:After treatment,the total effective rate was 92.50%in the trial group,higher than that of 75.00%in the control group(P<0.05).The MAS scores of wrist flexor muscle group,elbow flexor muscle group,and shoulder internal rotator muscle group in the two groups were decreased when compared with those before treatment(P<0.05),and the MAS scores of wrist flexor muscle group,elbow flexor muscle group,and shoulder internal rotator muscle group in the trial group were lower than those in the control group(P<0.05).The scores of FMA-UE and MBI in the two groups were increased when compared with those before treatment(P<0.05),and the scores of FMA-UE and MBI in the trial group were higher than those in the control group(P<0.05).Conclusion:Acupoint application and moxibustion combined with comprehensive rehabilitation therapy has a good clinical effect in treating upper limb dysfunction after stroke,which can reduce the muscle tension of the affected upper limb muscle groups,promote the recovery of upper limb function,and improve selfcare ability.
