肝硬化96例血清中微RNA-454和Wnt10a的表达及其临床意义

摘要 目的探究肝硬化病人血清中微RNA(microRNA,miR)-454和无翅型小鼠乳腺肿瘤病毒整合位点家族成员10a(Wnt10a)的表达及其临床意义。方法以2018年2月至2020年12月入住武汉科技大学附属汉阳医院的96例肝硬化病人为肝硬化组,并根据Child-Pugh分级分为A级36例、B级31例、C级29例。另选60例健康体检者为对照组。抽取空腹外周血检测天冬氨酸转氨酶(AST)、白蛋白(ALB)、丙氨酸转氨酶(ALT)等肝功能指标,qRT-PCR法检测血清中miR-454和Wnt10a表达水平,并分析miR-454、Wnt10a与Child-Pugh分级评分及AST、ALT、ALB等肝功能指标的关系;受试者操作特征(ROC)曲线分析miR-454、Wnt10a对严重肝硬化的诊断效能。结果肝硬化组miR-454表达水平0.85±0.20)显著低于对照组1.46±0.29,Wnt10a表达水平1.67±0.31显著高于对照组0.94±0.23(P<0.05);肝硬化组GGT、CLO、AST、ALP、ALT、TBil水平明显高于对照组,而ALB水平低于对照组(P<0.05);Child-Pugh A级、B级、C级中miR-454表达水平依次降低(P<0.05);Wnt10a在Child-Pugh A级、B级、C级中表达水平逐渐增加(P<0.05);血清miR-454与Wnt10a表达水平呈负相关(r=-0.53,P<0.001);血清miR-454表达水平与ALB呈正相关(r>0,P<0.05),与GLO、TBil、AST、GGT、Child-Pugh分级、ALT、ALP呈负相关(r<0,P<0.05);血清Wnt10a表达水平与ALB呈负相关(r<0,P<0.05),与GLO、TBil、AST、GGT、Child-Pugh分级、ALT、ALP呈正相关(r>0,P<0.05);miR-454、Wnt10a诊断严重性肝硬化的曲线下面积分别为0.80[95%CI:(0.71,0.88)]、0.73[95%CI:(0.621,0.837)];miR-454与Wnt10a联合诊断的曲线下面积为0.84[95%CI:(0.75,0.93)]。结论肝硬化病人血清中miR-454下调表达,Wnt10a上调表达,miR-454和Wnt10a呈负相关性,二者均与肝功能损伤有关,有作为诊断肝硬化严重程度的潜能。 Objective To investigate the expression and clinical significance of microRNA(miR)-454 and wingless-type mouse mammary tumor virus integration site family,member 10A(Wnt10a)in the serum of patients with liver cirrhosis and its clinical significance.Methods A total of 96 patients with cirrhosis admitted to Hanyang Hospital Affiliated to Wuhan University of Science and Technolo‑gy from February 2018 to December 2020 were selected as the cirrhosis group,and they were assigned into 36 cases in grade A,31 cas-es in grade B,and 29 cases in grade C according to the Child-Pugh grading.Another 60 healthy subjects were selected as the control group.The fasting peripheral blood was taken to detect liver function indicators such as aspartate aminotransferase(AST),albumin(ALB),alanine aminotransferase(ALT),etc.The expression levels of miR-454 and Wnt10a in serum were detected with qRT-PCR method.The relationship between miR-454,Wnt10a and Child-Pugh grading score and liver function indicators,such as AST,ALT,ALB,was analyzed;the diagnostic efficacy of miR-454 and Wnt10a for severe liver cirrhosis was analyzed with ROC curve.Results The expression level of miR-454 in the cirrhosis group 0.85±0.20 was significantly lower than that in the control group 1.46±0.29,and the expression level of Wnt10a 1.67±0.31 was significantly higher than that in the control group 0.94±0.23(P<0.05);the levels of gluta-myltransferase(GGT),globulin(CLO),AST,ALP,ALT,and total bilirubin(TBil)in the liver cirrhosis group were significantly higher than those in the control group,while the level of ALB was lower than that in the control group(P<0.05);the expression level of miR-454 in Child-Pugh grade A,B,and C decreased sequentially(P<0.05);the expression level of Wnt10a in Child-Pugh grade A,B,and C in-creased gradually(P<0.05);serum miR-454 was negatively correlated with the expression level of Wnt10a(r=−0.53,P<0.001);the ex-pression level of serum miR-454 was positively correlated with ALB(r>0,P<0.05),and negatively correlated with GLO,TBil,AS
