摘要
目的 分析腰椎管狭窄症(DLSS)患者应用中医正骨法与单侧双通道内镜手术联合治疗的效果。方法 选取2021年1月至2023年1月郑州市骨科医院收治的92例DLSS患者为研究对象,按随机数字表法分为对照组与研究组,每组46例。对照组行单通道内镜手术治疗,研究组行单侧双通道内镜手术联合中医正骨法治疗。比较两组临床疗效、疼痛程度[视觉模拟评分法(VAS)评分]、腰椎功能[日本骨科协会量表(JOA)评分、Oswestry功能障碍指数(ODI)评分]及炎性因子[白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)及白细胞介素-6(IL-6)]水平。结果 研究组总有效率(95.65%,44/46)比对照组高(80.43%,37/46),差异有统计学意义(P<0.05)。治疗后1个月、3个月VAS评分比较,研究组低于对照组(P<0.05)。研究组治疗后JOA评分比对照组高(P<0.05)。研究组治疗后ODI评分低于对照组(P<0.05)。治疗后IL-1β、TNF-α及IL-6水平比较,研究组低于对照组(P<0.05)。结论 应用中医正骨法与单侧双通道内镜手术联合治疗,可促进腰椎功能恢复,有利于减轻疼痛,并下调炎性因子表达。
Objective To analyze the effect of traditional Chinese medicine bone setting method combined with unilateral dual channel endoscopic surgery on patients with lumbar spinal stenosis(DLSS).Methods Ninety⁃two patients with DLSS admitted to Zhengzhou Orthopedic Hospital from January 2021 to January 2023 were selected,and divided into two groups by a random number table,with 46 cases in each group.The control group underwent single channel endoscopic surgery,while the study group underwent single side dual channel endoscopic surgery combined with traditional Chinese medicine bone setting therapy.The clinical efficacy,pain level(VAS score),lumbar function(JOA score and ODI score),and inflammatory factors[interleukin⁃1β(IL⁃1β),tumor necrosis factor⁃α(TNF⁃α)and interleukin⁃6(IL⁃6)]of the two groups were compared.Results The total effective rate of the study group(95.65%,44/46)was significantly higher than that of the control group(80.43%,37/46)(P<0.05).The VAS score of the study group was lower than that of the control group at 1 month and 3 months after treatment(P<0.05).The JOA score of the study group was higher than that of the control group after treatment(P<0.05).After treatment,the ODI score of the study group was lower than that of the control group(P<0.05).After treatment,the levels of Il⁃1β,TNF⁃αand IL⁃6 in the study group were lower than those in the control group(P<0.05).Conclusions The combination of traditional Chinese medicine bone setting method and unilateral dual channel endoscopic surgery can promote the recovery of lumbar function,alleviate pain,and downregulate the expression of inflammatory factors.
