摘要
目的探究爱普斯坦-巴尔病毒(epstein-barr virus,EBV)感染儿童外周血辅助性T细胞(helper T cell,Th)1/Th2型细胞标志物表达水平变化,分析其临床意义。方法选取2022年1月—2023年1月河南省儿童医院(郑州儿童医院)收治的EBV感染患儿64例,将其纳入EBV组,统计EBV组的EBV-DNA载量,根据中位水平将EBV组分为高EBV组和低EBV组,另外纳入64例健康儿童作为健康组,比较各组的Th1/Th2型细胞标志物[干扰素-γ(interferon-γ,IFN-γ)、白细胞介素-2(interleukin-2,IL-2)、白细胞介素-4(interleukin-4,IL-4)、白细胞介素-6(interleukin-6,IL-6)]表达水平和Th1/Th2比值差异,采用Spearson相关性分析法分析EBV-DNA病毒载量与Th1/Th2型细胞标志物水平的相关性。结果EBV组的IFN-γ、IL-2、IL-4、IL-6、Th1/Th2高于健康组,差异有统计学意义(P<0.001);高EBV组的IFN-γ、IL-2、Th1/Th2低于低EBV组,IL-4、IL-6水平高于低EBV组,差异有统计学意义(P<0.05)。Spearson分析结果显示,EBV-DNA病毒载量与IFN-γ、IL-2、Th1/Th2呈负相关性(r=-0.575、-0.586、-0.475,P<0.05),与IL-4、IL-6呈正相关性(r=0.462、0.437,P<0.05)。结论EBV感染患儿的IFN-γ、IL-2、IL-4、IL-6等Th1/Th2型细胞标志物表达水平发生明显变化,并且随着病毒载量的升高,Th1/Th2失衡加重,这对判断病情进展、指导临床策略制定具有积极意义。
Objective To investigate the changes in the expression levels of Th1/Th2 cell markers in peripheral blood of children infected with Epstein-Barr virus(EBV)and to analyze their clinical significance.Methods 64 children infected with EBV admitted to Henan Children's Hospital(Zhengzhou Children's Hospital)from January 2022 to January 2023 were selected and included in the EBV group,and the EBV-DNA load of the EBV group was counted.The EBV group was categorized into the high EBV group and the low EBV group according to the median level,and another 64 healthy children were included as the healthy group.The expression levels of Th1/Th2 type cell markers[interferon-γ(IFN-γ),interleukin-2(IL-2),interleukin-4(IL-4),and interleukin-6(IL-6)]and the differences in Th1/Th2 ratio were compared between the groups.Spearson correlation analysis was used to analyze the correlation between EBV-DNA viral load and the levels of Th1/Th2 type cell markers.Results IFN-γ,IL-2,IL-4,IL-6,and Th1/Th2 in the EBV group were higher than those in the healthy group,and the difference was statistically significant(P<0.001).The levels of IFN-γ,IL-2 and Th1/Th2 in high EBV group were lower than those in low EBV group,and the levels of IL-4 and IL-6 were higher than those in low EBV group,and the difference was statistically significant(P<0.05).Spearson analysis showed that EBV-DNA viral load was negatively correlated with IFN-γ,IL-2,and Th1/Th2(r=-0.575,-0.586,-0.475,P<0.05),and positively correlated with IL-4 and IL-6(r=0.462,0.437,P<0.05).Conclusion The expression levels of Th1/Th2 type cellular markers such as IFNγ,IL-2,IL-4,IL-6 and other cellular markers in children infected with EBV changed significantly,and the Th1/Th2 imbalance increased with the elevation of viral load,which is positively significant for judging the progress of the disease and guiding the development of clinical strategies.
