摘要 目的探讨增强型体外反搏(EECP)治疗老年稳定性冠心病(SCAD)患者的临床效果及对心脏重塑、神经内分泌因子和心率减速力(DC)的影响。方法选取2019年5月—2022年5月至成都市第三人民医院心内二科就诊的老年SCAD患者110例,根据信封法随机分成2组:增强型体外反搏(EECP)组(n=55)和对照组(n=55)。对照组予常规药物治疗,EECP组在对照组基础上增加EECP治疗。比较2组治疗前后心室重构指标、神经内分泌因子水平、DC及治疗后临床疗效、不良反应。结果研究期间脱落3例,EECP组完成55例,对照组完成52例,最终共107例患者纳入本研究。治疗后,EECP组左室收缩末期内径(LVESD)和容积(LVESV)、左室舒张末期内径(LVEDD)和容积(LVEDV)数值及PTH、AngⅡ、ALD、PRA水平均明显低于对照组(P<0.05);EECP组DC、临床疗效显著高于对照组(P<0.05);2组患者不良反应发生率差异无统计学意义(P>0.05)。结论使用EECP治疗老年SCAD,疗效显著,可有效延缓心室重构,改善神经内分泌因子水平及DC,具有一定的应用价值。 Objective To investigate the clinical effects of enhanced extracorporeal counterpulsation(EECP)in the treatment of elderly patients with stable coronary artery disease(SCAD)and its effects on cardiac remodeling,neuroendocrine factors and deceleration capacity of rate(DC).Methods A total of 110 elderly patients with SCAD who visited Second Department of Cardiology,Third People s Hospital of Chengdu City from May 2019 to May 2022 were selected and randomly divided into 2 groups according to envelope method:EECP group(n=55)and control group(n=55).The control group received conventional drug treatment,while the EECP group received EECP treatment in addition to the conventional drug treatment.The ventricular remodeling indexes,neuroendocrine factor level,DC,clinical efficacy and adverse reactions were compared between the two groups before and after treatment.Results There were 3 cases lost during the study.Finally,a total of 107 patients were included in this study,55 ones in the EECP group and 52 ones in the control group.After treatment,the values of left ventricular end-systolic diameter(LVESD)and volume(LVESV),left ventricular end-diastolic diameter(LVEDD)and volume(LVEDV)and the levels of PTH,AngⅡ,ALD and PRA of the EECP group were significantly smaller/lower than those of the control group(P<0.05).The DC and clinical efficacy of the EECP group were significantly higher than those of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion EECP is effective in the treatment of SCAD in elderly patients,which can effectively delay ventricular remodeling,improve the level of neuroendocrine factors and DC,and has certain application value.
