摘要
目的通过对2015-2020年郑州市疑似预防接种异常反应(AEFI)监测结果的分析,评价郑州市AEFI监测系统的运转状况及预防接种的安全性。方法通过中国免疫规划信息管理系统和全民健康保障信息化工程中国疾病预防控制信息系统收集2015-2020年郑州市报告的AEFI个案数据,采用描述性流行病学方法分析AEFI相关指标的报告情况。结果2015-2020年郑州市共报告AEFI个案7392例,年均报告发生率24.65/10万剂,其中不良反应占98.94%(7314例,一般反应和异常反应分别占93.11%和5.83%),偶合症占0.95%,心因性反应占0.07%,接种事故占0.04%。2017-2020年各项监测指标均达到了年度指标要求,但2015年48 h内调查率(89.36%)、3 d内调查表上传率(32.98%)、7 d内调查报告上传率(69.70%)和2016年3 d内调查表上传率(82.50%)、7 d内调查报告上传率(66.10%)没有达到指标要求。报告AEFI个案数居前3位的是金水区、管城区和二七区,共报告3704例,占总数的50.11%;男性占56.98%,女性占43.02%(χ2=98.22,P<0.001);3岁以下儿童占85.43%。报告发生率居前3位的疫苗依次为麻风疫苗(113.87/10万剂)、13价肺炎疫苗(80.59/10万剂)、DTaP-Hib四联疫苗(73.18/10万剂)。临床诊断中,发热/红肿/硬结等症状诊断占92.37%(发生率为22.77/10万剂),过敏性反应占5.02%(发生率为1.24/10万剂)。结论2015-2020年郑州市AEFI监测质量逐步提高,疫苗预防接种的安全性良好,报告的AEFI以一般反应为主,集中在3岁以下年龄组。
Objective To analyze the results of adverse event following immunization(AEFI)surveillance in Zhengzhou city from 2015 to 2020 for evaluating the operation of AEFI surveillance system and the safety of vaccination.Methods The AEFI case data in Zhengzhou city from 2015 to 2020 were collected through the China Immunization Information System and China Disease Prevention and Control Information System,National Health Insurance Information Technology Project.The descriptive epidemiological method was used for analyzing the relevant indicators.Results A total of 7392 AEFI cases were reported in Zhengzhou city from 2015 to 2020,with the average reported incidence of 24.65 cases per 105 doses.0f the total cases,93.11%were common vaccine reactions:5.83%were rare vaccine reactions;0.95%were coupled disease;0.07%were psychogenic reaction;0.04%were inoculation accident.All monitoring indicators from 2017 to 2020 met the requirements of the annual indicators,but in 2015 the proportion of cases having been investigated within 48 hours(89.36%),the proportion of cases with uploaded investigation form within 3 days(32.98%)and the proportion of cases with uploaded investigation report within 7 days(69.70%),and in 2016 the proportion of cases with uploaded form within 3 days(82.50%)and the proportion of cases with uploaded report within 7 days(66.10%)did not meet the target requirements.Of the total cases,50.11%(3704)AEFI cases were reported from Jinshui district,Guancheng district and Erqi district,which were ranked as the top three districts with highest number of cases.Males and females accounted for 56.98%and 43.02%of the total cases(X=98.22,P<0.001),respectively;children aged 3 years or younger accounted for 85.43%of the total cases.The top three AEFI incidence was resulted from leprosy vaccine(113.87 per 105 doses),13 valent pneumonia vaccine(80.59 per 105 doses)and DTaP-Hib vaccine(73.18 per 10s doses).Of the clinical diagnoses,92.37%were fever/red and swollen/induration with the incidence of 22.77 per 1105 doses,and 5.02%w
