摘要
患者女,67岁,面部及躯干红斑、水疱伴瘙痒1个月。皮肤科检查:头面部、躯干散在红斑、水疱,颜色鲜红至暗红,局部可见水疱破溃后遗留鲜红剥蚀面,上覆棕褐色结痂,尼氏征(+)。临床表现及病理符合寻常型天疱疮。治疗方案:予甲泼尼龙20 mg治疗12天后减量,联合应用阿普米司特片30 mg每日两次控制病情。随访3个月皮疹未见复发。
A 67-year-old female presented with facial and trunk erythema,blisters,and itching for 1 month.Dermatological examination:erythema and blisters were scattered on the head,face and trunk,and the color was bright red to dark red.There was a bright red denuded surface left after the blister has been broken locally,covered with brown scab,Nikolsky's sign(+).The clinical feature and pathology were consistent with pemphigus vulgaris.The patient was administrated with methylprednisolone 20 mg per day for 12 days and then the dosage was reduced,and the combination of 30 mg of apremilast tablets were administered twice a day to control the condition.No recurrence occurred in a 3-month follow-up.
