高同型半胱氨酸血症与糖尿病相关心脑血管损害的研究 被引量:1

摘要 目的 探讨血浆同型半胱氨酸与T2DM患者继发心脑血管疾病的相关性,为临床诊疗提供理论依据。方法 选取2020年5月-2022年2月本院收治的160例T2DM患者,收集患者血压、身高、体重、HCY、C肽、血糖、肝功能、肾功能等指标,分为高HCY组和正常HCY组,进行组间观察指标及UKPDS评分比较。Logistic回归分析心脑血管风险、血浆HCY及其易患因素的相关性。结果 高HCY组患者的年龄、病程、BMI、DBP、SBP、FBG、2hPG、HbA1c、TG、TC均偏高(P<0.05),GFR偏低(P<0.05);性别、肝功能、BUN、Scr、HDL-C、LDL-C无显著性差异(P>0.05);高HCY组的冠心病风险指数、致死性冠心病风险指数、卒中风险指数、致死性卒中风险指数明显偏高(P<0.05);HbA1c、TG、TC、HCY、病程是T2DM患者继发心脑血管疾病的危险因素(P<0.05)。结论 高同型半胱氨酸血症是T2DM患者继发心脑血管疾病的独立危险因素,应予纠治。 Objective To explore the correlation between plasma homocysteine and secondary cardio-cerebrovascular diseases in patients with type II diabetes so as to provide theoretical basis for clinical diagnosis and treatment.Methods 160 patients with type II diabetes admitted to our hospital from May 2020 to February 2022 were selected.Blood pressure,height,weight,Hcy,C-peptide,blood glucose,liver function,renal function and other clinical indicators were collected.Then they were divided into high HCY group and normal HCY group.Performed inter-group observation indicators and the UK Prospective diabetes Study(UKPDS)score were compared.Cardio-cerebrovascular risk,plasma HCY level and its risk factors was analyzed by logistic regression.Results The age,course of disease,BMI,DBP,SBP,FBG,2hPG,HbA1c,TG,TC of patients in the high HCY group were all higher(P<0.05),while GFR was lower(P<0.05);There was no significant difference in gender,liver function,BUN,Scr,HDL-C,LDL-C(P>0.05);The coronary heart disease risk index,fatal coronary heart disease risk index,stroke risk index and fatal stroke risk index of the high HCY group were significantly higher(P<0.05);HbA1c,TG,TC,HCY and course of disease were the risk factors of secondary cardio cerebral vascular disease in type II diabetes patients(P<0.05).Conclusions Hyperhomocysteinemia is an independent risk factor for cardiovascular and cerebrovascular diseases secondary to type II diabetes mellitus,which should be corrected and treated.
