摘要
目的:研究五味化痔胶囊对痔模型大鼠肛周肿胀、溃疡、出血等症状,直肠组织病理形态学,炎症因子,磷脂酰肌醇3-激酶/蛋白激酶B/核因子κB(PI3K/AKT/NF-κB)信号通路表达的影响,探究其治疗痔病的作用机制。方法:SD大鼠分为空白组、模型组、美沙拉嗪组、五味化痔胶囊低、中、高剂量组。选用角叉菜胶、脂多糖、山莨菪碱、酚磺乙胺混合液,注射于大鼠肛肠局部复制大鼠痔模型。造模后,中药各组予五味化痔胶囊干预7 d,观察肛周肿胀、溃疡、出血等症状表现,HE染色观察直肠组织病理形态,酶联免疫吸附试验检测组织匀浆中炎性因子肿瘤坏死因子-α(TNF-α)、白介素1β(IL-1β)以及白介素-6(IL-6)的水平,qRT-PCR技术观察直肠组织中PI3K、AKT、NF-κB的基因表达,免疫组化法观察直肠组织环氧化酶2(COX2)及诱导型一氧化氮合酶(iNOS)蛋白表达。结果:与空白组相比,模型组肛周肿胀、溃疡、出血等症状评分升高(P<0.05),光镜下观察到直肠组织黏膜缺损、间质血管扩张充血、炎性细胞浸润等情况;血清IL-1β、IL-6和TNF-α水平上升(P<0.05),直肠组织中PI3K、AKT、NF-κB的mRNA表达及COX2、iNOS蛋白表达升高(P<0.05)。与模型组相比,美沙拉嗪组和五味化痔胶囊高、中剂量组症状评分下降(P<0.05),直肠组织黏膜缺损、间质血管扩张充血、炎性细胞浸润等情况有所缓解;美沙拉嗪组和五味化痔胶囊高、中、低剂量组IL-1β、IL-6和TNF-α水平下降(P<0.05),PI3K、NF-κB mRNA表达和COX2、iNOS蛋白表达下降(P<0.05),五味化痔胶囊高剂量组AKT mRNA表达下降(P<0.05)。结论:五味化痔胶囊可能通过抑制PI3K/AKT/NF-κB通路、调节炎症因子的水平治疗痔病。
Objective:To study the effects of Wuwei Huazhi Capsule on perianal swelling,ulcers,bleeding and other symptoms,rectal tissue pathomorphology,inflammatory factors,phosphatidylinositol 3-kinase/protein kinase B/nuclear factorsκB(PI3K/AKT/NF-κB)signaling pathway expression in hemorrhoid model rats,and its mechanism of action in treating hemorrhoids.Methods:SD rats were divided into blank group,model group,Mesalazine group,and low,medium,and high-dose groups of Wuwei Huazhi Capsule.Select a mixture of carrageenan,lipopolysaccharide,scopolamine,and ethylsulfonamide to inject into the local rectum of rats to replicate a rat hemorrhoid model.After successful modeling,Wuwei Huazhi Capsules were administered by gavage for 7 days to observe symptoms such as perianal swelling,ulcers,and bleeding.HE staining was used to observe the pathological morphology of rectal tissue,enzyme-linked immunosorbent assay to detect the inflammatory factor TNF-α,Interleukin 1β(IL-1β),and the levels of IL-6,qRT PCR technology to observe the gene expressions of PI3K,AKT,NF-κB,and immunohistochemistry to observe the protein expression of cyclooxygenase-2(COX2)and inducible nitric oxide synthase(iNOS)in rectal tissue.Results:Compared with the blank group,the model group showed an increase in symptoms such as perianal swelling,ulcers,and bleeding(P<0.05),and rectal tissue mucosal defects,interstitial vascular dilation and congestion,and inflammatory cell infiltration were observed under the light microscope;IL-1β,IL-6 and TNF-αin serum increased(P<0.05),PI3K,AKT,NF-κB expressions in rectal tissue as well as the expressions of COX2 and iNOS proteins increased(P<0.05);Compared with the model group,the symptom scores of Mesalazine group and the high and all Wuwei Huazhi Capsule groups decreased(P<0.05),and the mucosal defects in rectal tissue,interstitial vascular dilation and congestion,and inflammatory cell infiltration were alleviated;IL-1β,IL-6 and TNF-αin the Mesalazine group and Wuwei Huazhi Capsules groups decreased(P<0.05),PI3K,AKT
