摘要
目的观察电针辨证治疗联合循经刮痧治疗紧张型头痛(TTH)的临床疗效。方法将120例TTH患者按照随机数字表法分为3组,均予盐酸阿米替林片口服治疗。电针组40例予电针辨证治疗,刮痧组40例予循经刮痧治疗,联合组40例予电针辨证治疗联合循经刮痧治疗,3组疗程均为8周。比较3组疗效;比较3组治疗前后头痛相关指标疼痛视觉模拟评分(VAS)、头痛发作次数、头痛发作持续时间;比较3组治疗前后汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD)评分、匹兹堡睡眠质量指数量表(PSQI)评分;比较3组治疗前后大脑中动脉(MCA)、前动脉(ACA)、后动脉(PCA)血流速度。结果电针组总有效率75.0%(30/40),刮痧组总有效率72.5%(29/40),联合组总有效率92.5%(37/40),联合组疗效优于电针组、刮痧组(P<0.05),电针组与刮痧组疗效比较差异无统计学意义(P>0.05)。3组治疗后疼痛VAS、头痛发作次数、头痛发作持续时间均较本组治疗前降低(P<0.05),治疗后联合组疼痛VAS、头痛发作次数、头痛发作持续时间均低于电针组、刮痧组(P<0.05),电针组与刮痧组疼痛VAS、头痛发作次数、头痛发作持续时间比较差异均无统计学意义(P>0.05)。3组治疗后HAMA评分、HAMD评分、PSQI评分均较本组治疗前降低(P<0.05),联合组治疗后HAMA评分、HAMD评分、PSQI评分均低于电针组、刮痧组(P<0.05),电针组与刮痧组HAMA评分、HAMD评分、PSQI评分比较差异均无统计学意义(P>0.05)。3组治疗后MCA、ACA、PCA血流速度均较本组治疗前降低(P<0.05),联合组治疗后MCA、ACA、PCA血流速度均低于电针组、刮痧组(P<0.05),电针组与刮痧组MCA、ACA、PCA血流速度比较差异无统计学意义(P>0.05)。结论电针辨证治疗联合循经刮痧治疗TTH,可明显改善患者心理情绪、睡眠质量和脑血流速度,减轻头痛症状。
Objective To observe the clinical effect of dialectical electric-acupuncture(EA)combined with scraping along meridians in treatment of tension type headache(TTH).Methods A total of 120 patients with TTH were selected as the research object,all patients were managed with oral amitriptyline hydrochloride,they were randomly assigned in 1∶1∶1 radio to EA group,scraping group,or combined group(dialectical EA+scraping along meridianson therapy).The treatment was performed for 8 weeks,the aim was to compare the curative effects,Visual analogue scale(VAS),the number and duration of headache attacks,the score of Hamilton anxiety rating/Hamilton depression rating(HAMA/HAMD),Pittsburgh Sleep Quality Index(PSQI),and the blood flow velocities of middle cerebral artery(MCA),anterior cerebral artery(ACA)and posterior cerebral artery(PCA)among the three groups before and after treatment.Results The total effective rate in the combined group was better than that in the EA group,the scraping group(92.5%[37/40]vs 75.0%[30/40]vs 72.5%[29/40],P<0.05),while the curative effect in EA group was similar to that in scraping group(P>0.05).After treatment,VAS,the number and duration of headache attacks in the three groups were significantly shorter than those before treatment(P<0.05),which were significantly shorter in the combined group than those in the EA group and the scraping group(P<0.05),but the EA group were comparable to the scraping group(P>0.05).The scores of HAMA/HAMD and PSQI,and the levels of MCA,ACA and PCA in the three groups after treatment were significantly lower than those before treatment(all P<0.05),the decrease was more pronounced in the combined group than in the EA group and the scraping group(all P<0.05),but there was no significant difference in the above indexes between the EA group and the scraping group(all P>0.05).Conclusion The treatment of dialectical EA combined with scraping along meridians for the patients with TTH has a significant effect.It can significantly improve the patient's psychological mood,
