摘要
目的:研究优化护理联合保温毯对胸腔镜肺癌根治术(RRPC)预后的影响并予以分析。方法:选取2020年1月—2022年1月,于莆田市第一医院拟行胸腔镜RRPC治疗的80例患者开展研究。采取随机数字表法将80例患者分为A组与B组,各40例。B组开展常规护理,A组则于B组基础上开展优化护理联合保温毯干预。分析两组预后、应激反应、术后并发症及自我感受负担的差异。结果:A组各项预后指标(下床活动时间、引流管拔出时间、住院时间、切口愈合时间)均优于B组(P<0.05)。术前两组各项应激反应指标(收缩压、舒张压、心率)水平差异均无统计学意义(P>0.05);术毕A组各项应激反应指标水平均低于B组(P<0.05)。A组并发症发生率为5.00%,低于B组的20.00%(P<0.05)。干预前,两组各项自我感觉负担量表(SPBS,情感、身体、经济)评分差异均无统计学意义(P>0.05);干预后,A组各项SPBS评分均低于B组(P<0.05)。结论:优化护理联合保温毯在改善胸腔镜RRPC患者预后方面具有一定优势,并发症发生率更低,减轻应激反应,改善自我感觉负担水平。
Objective:To study and analyze the effect of optimal nursing combined with thermal blanket on the prognosis of thoracoscopic radical resection of pulmonary carcinoma(RRPC).Method:A total of 80 patients who underwent thoracoscopic RRPC treatment in the First Hospital of Putian City from January 2020 to January 2022 were selected for this study.Eighty patients were divided into group A and group B by random number table method,40 cases in each group.Group B carried out routine nursing,group A carried out optimal nursing combined with thermal blanket intervention on the basis of group B.The differences of prognosis,stress response,postoperative complications and perceived burden between the two groups were analyzed.Result:The prognosis indicators(time to get out of bed,drainage tube removal time,hospitalization time,incision healing time)in group A were better than those in group B(P<0.05).There were no significant differences in stress response indexes(systolic blood pressure,diastolic blood pressure,heart rate)between the two groups before operation(P>0.05).After operation,the stress response indexes in group A were lower than those in group B(P<0.05).The incidence of complications in group A was 5.00%,which was lower than 20.00%in group B(P<0.05).Before intervention,there were no statistically significant differences in the scores of self-perceived burden scale(SPBS,emotional,physical,economic)between the two groups(P>0.05).After intervention,the scores of SPBS in group A were lower than those in group B(P<0.05).Conclusion:Optimal nursing combined with thermal blanket has certain advantages in improving the prognosis of thoracoscopic RRPC patients,with lower complication rate,reduced stress response and improved self-perceived burden level.
