摘要
目的:探讨以术前预后营养指数(PNI)为基础的个体化营养支持对心脏手术患者营养状况及临床结局的影响,为改善患者营养状况提供依据。方法:选取2020年8月—2022年7月福建医科大学附属协和医院收治的104例实施心脏手术的患者作为研究对象,按照随机数表法将其分为对照组与研究组,每组各52例。对照组实施常规护理,研究组在常规护理基础上实施以术前PNI为基础的个体化营养支持干预。比较两组患者的营养状况[白蛋白(Alb)、前白蛋白(PAB)、血红蛋白(Hb)、转铁蛋白(TF)水平]、康复进程、并发症及生活质量[生活质量综合评定问卷(GQOLI-74)评分]情况。结果:术后5 d、10 d,研究组患者Alb、PAB、Hb、TF水平均高于对照组,差异有统计学意义(t=7.255、8.979、3.379、6.002、4.304、2.587、3.693、4.351,P<0.05)。研究组患者机械通气时间、ICU入住时间、引流管拔除时间、住院时间均明显短于对照组,差异有统计学意义(t=7.708、6.019、3.857、5.130,P<0.05)。研究组患者并发症发生率明显低于对照组,差异有统计学意义(χ^(2)=4.981,P<0.05)。术后10 d,研究组患者GQOLI-74问卷各维度评分明显高于对照组,差异有统计学意义(t=5.297、5.839、7.972、10.437,P<0.05)。结论:以术前PNI为基础的个体化营养支持干预,能够改善心脏手术患者围手术期营养状况及临床结局,促进患者术后康复,降低术后并发症发生风险,提高患者的生活质量。
Objective:To investigate the effect of individualized nutritional support based on preoperative prognostic nutritional index(PNI)on the nutritional status and clinical outcomes of cardiac surgery patients,and to provide a basis for improving the nutritional status of patients.Methods:104 patients who underwent cardiac surgery in the hospital from August 2020 to July 2022 were selected as the study subjects,and they were divided into the control group and the study group according to the random number table method,with 52 cases in each group.The control group implemented conventional nursing care,and the study group implemented individualized nutritional support intervention based on preoperative PNI on the basis of conventional nursing care.The nutritional status(albumin[Alb],prealbumin[PAB],hemoglobin[Hb],transferrin[TF]levels),recovery process,complications,and quality of life(score of the Comprehensive Quality of Life Assessment Questionnaire[GQOLI-74])were compared between the two groups.Results:At 5 d and 10 d postoperatively,the levels of Alb,PAB,Hb,and TF of patients in the study group were higher than those of the control group,and the difference was statistically significant(t=7.255,8.979,3.379,6.002,4.304,2.587,3.693,4.351,P<0.05).The mechanical ventilation time,ICU stay,drain removal time,and hospitalization time of the patients in the study group were significantly shorter than those in the control group,and the differences were statistically significant(t=7.708,6.019,3.857,5.130,P<0.05).The complication rate of patients in the study group was significantly lower than that of the control group,and the difference was statistically significant(χ^(2)=4.981,P<0.05).At 10 d postoperatively,the scores of the dimensions of the GQOLI-74 questionnaire of the patients in the study group were significantly higher than those of the control group,and the difference was statistically significant(t=5.297,5.839,7.972,10.437,P<0.05).Conclusion:Individualized nutritional support interventions based on preoperative PNI
