摘要
目的探讨基于云平台的远程超声引导下血管通路技术继续教育模式的效果以及学员的满意度。方法选择2020年1月—2022年6月在国内12家血管通路中心进修学习的39名进修医师为研究对象,通过云平台与其进修时期相应的指导医师连线,完成当地医院持续血液透析患者通路评估以及超声引导下经皮血管腔内成形术(percutaneous transluminal angioplasty,PTA)。采用进修医师操作过程评分表和进修医师满意度问卷评价培训效果。结果对进修医师操作全程的考核发现,血管评估技能平均得分(88.24±4.31)分,其中内科情况评估、透析情况评估、通路情况评估以及手术方案制定分别得分:(14.29±0.52)分、(21.03±1.72)分、(37.53±1.43)分、(15.40±2.51)分。超声引导下PTA相关技能平均得分(88.56±3.62)分,其中无菌观念、人文关怀、操作熟练程度满分人数较多,分别占总人数的94.87%、82.05%、56.41%。操作前准备、术中准备、操作后注意事项得分相对较高,分别为(18.18±1.35)分、(15.00±0.92)分、(13.92±1.04)分。主要的失分项为术中超声操作、导丝操作以及球囊操作。满意度调查发现,所有进修医师均对基于云平台的远程超声引导下血管通路技术教育模式表示满意,认为云平台指导可激发学习热情、增强自主学习能力,提高沟通和语言表达能力等,提高超声引导下血管通路技术水平。结论基于云平台的超声引导下血管通路技术继续医学教育模式可为进修医师提供个体化、精准化指导,进修医师对该模式的满意度高,可提高医师学习效率,缩短学习周期,增强其在当地开展该技术的信心,促进多中心超声引导下血管通路技术的普及化。
Objective To explore the effect of the continuing education mode of vascular access technology under the guidance of remote ultrasound based on the cloud platform and the satisfaction of students.Methods A total of 39 training physicians who studied at 12 vascular access centers in China from January 2020 to June 2022 were selected as the research subjects.Through the cloud platform and its corresponding guidance physicians during the study period,they completed the assessment of continuous hemodialysis patient access in local hospitals and ultrasound guided percutaneous transluminal angioplasty(PTA).The training effect was evaluated by the operating process scoring scale of the training physicians and the satisfaction questionnaire of the training physicians.Results The examination of the whole process of the training physicians'operation showed that the average score of vascular evaluation skills was(88.24±4.31)points,including the scores of internal medicine evaluation,dialysis evaluation,access evaluation and surgical plan formulation:(14.29±0.52)points,(21.03±1.72)points,(37.53±1.43)points,and(15.40±2.51)points.The average score of PTA-related skills under ultrasound guidance was(88.56±3.62)points,and the number of full marks of aseptic concept,humanistic care and operation proficiency was relatively large,accounting for 94.87%,82.05%and 56.41%of the total respectively.The scores of preparations before the operation,preparation during the operation and precautions after the operation were relatively high,(18.18±1.35)points,(15.00±0.92)points,and(13.92±1.04)points,respectively.The main loss items were intraoperative ultrasound operation,guide wire operation and balloon operation.The satisfaction survey found that all the refresher physicians were satisfied with the long-distance ultrasound-guided vascular access technology education model based on the cloud platform and believed that the cloud platform guidance could stimulate learning enthusiasm,enhance independent learning ability,improve communica
