摘要
目的评价张氏杜仲补骨汤治疗肝肾亏虚型原发性骨质疏松症临床疗效。方法将90例肝肾亏虚型原发性骨质疏松症患者随机分成对照组、试验组各45例,对照组予以鲑鱼降钙素联合钙尔奇D,试验组在对照组基础上加用张氏杜仲补骨汤内服,治疗3个月后对比观察两组临床疗效、左侧股骨颈、腰椎L1-4骨密度值、血钙(Ca)、骨碱性磷酸酶(BALP)、骨钙素(BGP)、25-羟基维生素D3[25-(0H)D3]水平变化、中医症候各项评分及总积分。结果最终对照组45例,脱落1例,完成44例,试验组45例,脱落2例,完成43例,(1)治疗后中医证候总积分试验组低于对照组(P<0.05),其中腰背疼痛、腰膝酸软、下肢疼痛评分低于对照组(P<0.05),而下肢痿软、步履困难、目眩各项评分两组差异无统计学意义(P>0.05),(2)治疗后试验组患者在左侧股骨颈、腰椎骨密度、Ca、BGP、25-(0H)D3水平高于对照组(P<0.05),BALP水平低于对照组(P<0.05),(3)临床总有效率试验组93.02%高于对照组75%(P<0.05)。结论张氏杜仲补骨汤能有效地防治肝肾亏虚型原发性骨质疏松症患者骨密度下降、改善骨代谢生化指标水平及临床症状。
Objective To evaluate the clinical effect of ZHANG's Duzhong Bugu Decoction(张氏杜仲补骨汤)on primary osteoporosis of liver and kidney type deficiency type.Methods 90 patients with primary osteoporosis with deficiency of liver and kidney were randomly divided into control group and experimental group with 45 patients in each group.The control group was given salmon calcitonin combined with calcitric D,the experimental group was additionally given ZHANG's Duzhong Bugu Decoction on the basis of the control group.After 3 months of treatment,the clinical efficacy,bone mineral density values of left femoral neck and lumbar L1-4,changes in blood calcium(Ca),bone alkaline phosphatase(BALP),osteocalcin(BGP),25-hydroxyvitamin D3[25-(OH)D3]levels,scores of TCM symptoms and total scores of the two groups were compared and observed.Results Finally,there were 45 cases in the control group,1 case of shedding and 44 cases of completion,and 45 cases in the experimental group,2 cases of shedding and 43 cases of completion.(1)The total score of TCM syndrome after treatment in experimental group was lower than that in control group(P<0.05).The scores of low back pain,low back and knee pain and lower limb pain were lower than those of control group(P<0.05).There was no significant difference in the scores of lower limb impotence,walking difficulty and vertigo between the two groups(P>0.05).(2)After treatment,the bone density,Ca,BGP,25-(OH)D3 levels in the left femoral neck and lumbar vertebrae of experimental group were higher than those in the control group(P<0.05),and the BALP level was lower than that in the control group(P<0.05).(3)The total clinical effective rate of experimental group was 93.02%higher than that of control group 75%(P<0.05).Conclusion ZHANG's Duzhong Bugu Decoction can effectively prevent and treat the decrease of bone mineral density and improve the biochemical indexes of bone metabolism and clinical symptoms in patients with primary osteoporosis of liver and kidney deficiency type.
