摘要
目的探讨急性心力衰竭患者并发急性肾损伤的危险因素。方法抽取2019年1月至2021年12月洛阳市中心医院收治的50例急性心力衰竭患者作为前瞻性研究对象。按照入院48 h内是否并发急性肾损伤将其分为发生组与未发生组。记录并比较两组基线资料及应激性高血糖发生状况,应用Logistic回归分析急性心力衰竭患者并发急性肾损伤的影响因素。结果50例急性心力衰竭患者中并发急性肾损伤16例(32%),未并发急性肾损伤34例(68%)。两组尿量、肌酐、入院时血糖水平比较差异有统计学意义(P<0.05);两组合并应激性高血糖、吸烟、贫血比例比较差异有统计学意义(P<0.05)。Logistic回归分析结果显示,合并应急性高血糖、吸烟、贫血是急性心力衰竭患者并发急性肾损伤的危险因素(OR=59.26、48.45、58.63,P均<0.05)。结论急性心力衰竭患者出现应激性高血糖容易并发急性肾损伤,吸烟、贫血对急性肾损伤的发生也有影响。
Objective To investigate risk factors for acute kidney injury in patients with acute heart failure.Methods Fifty patients with acute heart failure admitted to Luoyang Central Hospital from January 2019 to December 2021 were selected as research objects of the prospective trail.And the patients were divided into occurrence group and non-occurrence group according to whether they were complicated by acute kidney injury within 48 hours after admission.The baseline data and occurrence of stress hyperglycemia were recorded and compared between the two groups.Logistic regression analysis was used to analyze the influencing factors of acute kidney injury in patients with acute heart failure.Results Among the 50 patients with acute heart failure,16 cases(32%)were complicated by acute kidney injury,and 34 cases(68%)were not complicated by acute kidney injury.There were significant differences in urine volume,creatinine,and blood glucose level at admission between the two groups(P<0.05).There were significant differences in the proportions of patients with stress hyperglycemia,smoking,and anemia between the two groups(P<0.05).Results of logistic regression analysis showed that stress hyperglycemia,smoking and anemia were risk factors for acute kidney injury in patients with acute heart failure(OR=59.26,48.45,58.63;all P<0.05).Conclusions Acute heart failure patients who have stress hyperglycemia is prone to developing acute kidney injury.Smoking and anemia also have influence on the occurrence of acute kidney injury.
