摘要
目的探讨白噪声联合非营养性吸吮(NNS)对减轻早产儿视网膜病筛查疼痛的作用。方法抽取2022年8月至2023年3月洛阳市中心医院收治的80例接受视网膜病筛查的早产儿,以随机数字表法分为对照组与观察组,每组40例。对照组婴儿筛查时接受NNS,观察组婴儿接受白噪声联合NNS。比较两组婴儿筛查前后的生理指标[心率(HR)、呼吸频率(R)、体温(T)、血氧饱和度(SpO2)]及修订版早产儿疼痛评估量表(PIPP-R)评分。结果筛查后1 min,两组婴儿PIPP-R评分均高于筛查前(P均<0.05);筛查后5 min,两组婴儿PIPP-R评分均低于筛查后1 min(P均<0.05)。筛查后1、5 min,观察组PIPP-R评分均低于对照组(P均<0.05)。筛查后1 min,两组婴儿心率高于筛查前,血氧饱和度低于筛查前(P<0.05);筛查后5 min,两组婴儿心率均低于筛查后1 min,血氧饱和度高于筛查后1 min(P<0.05)。筛查后1、5 min,观察组心率低于对照组,血氧饱和度高于对照组(P<0.05)。结论白噪声联合NNS可以有效减轻早产儿在视网膜病筛查过程中的疼痛,提高筛查的舒适性,同时不会对婴儿的生理指标产生不良影响,可作为一种安全有效的疼痛干预方法。
Objective To study the effect of white noise combined with non-nutritive sucking(NNS)on reducing the pain in the screening for retinopathy in premature infants.Methods A total of 80 premature infants who underwent retinopathy screening in Luoyang Central Hospital from August 2022 to March 2023 were selected and divided into control group and observation group by random number table method,with 40 cases in each group.The control group received NNS during screening,and the observation group received white noise combined with NNS.The physical indicators,including heart rate(HR),respiratory frequency(R),body temperature(T)and oxygen saturation(SpO2),and premature infant pain profile-revised(PIPP-R)score of the two groups were compared before and after screening.Results Compared with indexes before screening,the PIPP-R score of the two groups increased at 1 minute after screening(all P<0.05).Compared with scores at 1 minute after screening,the PIPP-R score of the two groups decreased at 5 minutes after screening,and the differences were statistically significant(all P<0.05).At 1 and 5 minutes after screening,the PIPP-R score of the observation group was significantly lower than that of the control group(all P<0.05).In addition,HR in both groups increased at 1 minute after screening,and SpO2 decreased,compared with those before screening(P<0.05).Compared with indexes at 1 minute after screening,the HR of the two groups decreased,and the SpO2 increased at 5 minutes after screening(P<0.05).At 1 and 5 minutes after screening,the observation group had lower HR and higher SpO2,compared with the control group(P<0.05).Conclusions White noise combined with NNS can effectively reduce the pain in premature infants during retinopathy screening,and improve the comfortability of screening,without adverse effect on physiological indicators of infants.It can be used as a safe and effective method of pain intervention.
