摘要
目的:探讨布地奈德雾化吸入联合阿奇霉素序贯治疗肺炎支原体感染致慢性咳嗽患儿的临床效果。方法:选择2020年10月—2023年1月北京市石景山医院收治的200例肺炎支原体感染致慢性咳嗽患儿为研究对象,采用完全双盲法将患儿分为对照组和试验组,各100例。对照组行阿奇霉素序贯治疗,试验组行阿奇霉素序贯联合布地奈德雾化吸入治疗,观察两组症状程度、炎症因子及气道反应性变化情况,并比较两组用药效果及安全性。结果:治疗14 d后,两组咳嗽积分、白细胞介素-6(interleukin-6,IL-6)、白细胞介素-17(interleukin-17,IL-17)、白三烯(leukotriene,LT)、肿瘤坏死因子-α(tumor necrosis factor,TNF-α)、C反应蛋白(C-reactive protein,CRP)较治疗前下降,且试验组低于对照组,两组用力肺活量(forced vital capacity,FVC)、第1秒用力呼气容积(forced expiratory volume in one second,FEV1)、最大呼气流速峰值(peak expiratory flow,PEF)、最大呼气中段流量(maximal mid-expiratory flow curve,MMEF)较治疗前升高,且试验组高于对照组,差异有统计学意义(P<0.05)。试验组治疗总有效率高于对照组,差异有统计学意义(P<0.05);试验组不良反应发生率与对照组比较,差异无统计学意义(P>0.05)。结论:肺炎支原体感染致慢性咳嗽患儿应用阿奇霉素序贯联合布地奈德雾化吸入治疗,可减轻炎症反应,纠正气道高反应性,缓解咳嗽症状,提高疗效,且不良反应少而轻微。
Objective:To explore the clinical effect of Budesonide atomization inhalation combined with Azithromycin sequential treatment in chronic cough caused by Mycoplasma pneumoniae infection.Method:A total of 200 children with chronic cough caused by Mycoplasma pneumoniae infection who admitted to the Beijing Shijingshan Hospital from October 2020 to January 2023 were selected as the study subjects,the children were divided into the control group and the experimental group by complete double-blind method,with 100 cases in each group.The control group was given Azithromycin sequential treatment,and the experimental group was given Azithromycin sequential treatment combined with Budesonide atomization inhalation.The changes of symptoms,inflammatory factors and airway responsiveness of two groups were observed,and the efficacy and safety of two groups were compared.Result:After 14 d of treatment,the cough scores,interleukin-6(IL-6),interleukin-17(IL-17),leukotriene(LT),tumor necrosis factor-α(TNF-α)and C-reactive protein(CRP)in two groups were lower than those before treatment,and those in the experimental group were lower than those in the control group,the forced vital capacity(FVC),forced expiratory volume in one second(FEV1),peak expiratory flow(PEF)and maximal mid-expiratory flow curve(MMEF)of two groups were higher than those before treatment,and those in the experimental group were higher than those in the control group,the differences were statistically significant(P<0.05).The total effective rate of treatment in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the experimental group and the control group(P>0.05).Conclusion:Azithromycin sequential treatment combined with Budesonide atomization inhalation in the treatment of children with chronic cough caused by Mycoplasma pneumoniae infection can alleviate inflammatory reactions,correct airway hyperresponsiven
