摘要
目的探讨盐酸阿芬太尼注射液复合依托咪酯对老年高血压患者行无痛胃镜检查的麻醉效果及血流动力学影响。方法选取2021年1月至2023年1月于新泰市人民医院接受无痛胃镜检查的老年高血压患者240例作为研究对象,随机分为对照组与观察组,每组120例。对照组给予丙泊酚进行麻醉,观察组给予盐酸阿芬太尼注射液复合依托咪酯复合麻醉,比较两组患者血流动力学指标、麻醉效果、简易智力状态检查量表(MMSE)评分和不良反应发生率。结果麻醉前两组心率、平均动脉压、心脏指数比较差异无统计学意义(P>0.05);麻醉后、检查结束时,观察组心率、平均动脉压、心脏指数明显优于对照组(P<0.05);观察组麻醉优良率显著高于对照组(P<0.05);麻醉前两组MMSE评分比较差异无统计学意义(P>0.05);麻醉后12、24 h,观察组MMSE评分明显高于对照组(P<0.05);观察组不良反应发生率显著低于对照组(P<0.05)。结论盐酸阿芬太尼注射液复合依托咪酯用于老年高血压患者无痛胃镜检查中,血流动力学稳定性高,麻醉效果显著,可改善患者认知功能,且安全性较高。
Objective To explore the anesthetic effect and hemodynamic effects of alfentanil hydrochloride injection combined with Etomidate on elderly hypertensive patients undergoing painless gastroscopy.MethodsA total of 240 elderly hypertensive patients who received painless gastroscopy in Xintai People's Hospital from January 2021 to January 2023 were selected as the study objects and randomly divided into control group and observation group,with 120 cases in each group.The control group was anesthetised with propofol,and the observation group was anesthetised with afentanil hydrochloride injection combined with etomidate.Hemodynamic indexes,anesthetic effect,simple mental state Examination Scale(MMSE)score and incidence of adverse reactions were compared between the two groups.Results There were no significant differences in heart rate,mean arterial pressure and heart index between the two groups before anesthesia(P>0.05).After anesthesia and at the end of examination,heart rate,mean arterial pressure and heart index in observation group were significantly better than those in control group(P<0.05).The excellent and good rate of anesthesia in observation group was significantly higher than that in control group(P<0.05).There was no significant difference in MMSE scores between the two groups before anesthesia(P>0.05).At 12 and 24 h after anesthesia,MMSE scores in observation group were significantly higher than those in control group(P<0.05).The incidence of adverse reactions in observation group was significantly lower than that in control group(P<0.05).Conclusion The application of afentanil hydrochloride injection combined with etomidate in painless gastroscopy in elderly patients with hypertension has high hemodynamic stability,significant anesthetic effect,and can improve the cognitive function of patients with high safety.
