摘要
目的观察急性髓系白血病(AML)患者血清SALL4和CASP1表达情况,探讨其与AML临床病理特征及预后的关系。方法2017年11月—2019年11月郑州大学第一附属医院诊治AML患者120例为AML组,均根据AML分型给予标准化疗方案治疗;同期体检健康者120例为对照组。采用实时荧光定量PCR法检测2组血清SALL4、CASP1 mRNA相对表达量;比较不同临床病理特征AML患者血清SALL4、CASP1mRNA相对表达量。将AML患者根据血清CASP1、SALL4mRNA相对表达量均值分为CASP1高表达组(CASP1mRNA相对表达量≥1.01)65例和CASP1低表达组(CASP1mRNA相对表达量<1.01)55例,SALL4高表达组(SALL4mRNA相对表达量≥0.96)61例和SALL4低表达组(SALL4mRNA相对表达量<0.96)59例。随访3年,比较CASP1高、低表达组,SALL4高、低表达组3年总生存率;采用多因素Cox回归分析AML患者3年死亡的危险因素。结果(1)AML组血清SALL4、CASP1mRNA相对表达量(0.96±0.15、1.01±0.22)均高于对照组(0.81±0.11、0.78±0.16)(t=6.147,P<0.001;t=6.443,P<0.001)。(2)脾脏肿大、白细胞计数≥10×10^(9)/L的AML患者SALL4、CASP1mRNA相对表达量(1.02±0.11、1.04±0.11;1.16±0.13、1.12±0.13)分别高于无脾脏肿大、白细胞计数<10×10^(9)/L者(0.96±0.08、0.95±0.09;0.91±0.07、0.98±0.09)(P<0.05),不同年龄、性别、预后危险度分层、核仁磷酸蛋白突变类型、血小板计数、血红蛋白、骨髓原始细胞比率、髓外病变、CD34、法美英分型者SALL4、CASP1 mRNA相对表达量比较差异均无统计学意义(P>0.05)。(3)随访至2023年3月,SALL4高表达组[34.43%(21/61)]、CASP1高表达组[35.38%(23/65)]3年总生存率分别低于SALL4低表达组[57.63%(34/59)]、CASP1低表达组[58.18%(32/55)](χ^(2)=14.417,P<0.001;χ^(2)=12.283,P=0.037)。(4)脾脏肿大(HR=1.256,95%CI:1.008~1.564,P=0.042)、CASP1高表达(HR=1.412,95%CI:1.127~1.769,P=0.003)、SALL4高表达(HR=1.378,95%CI:1.081~1.757,P=0.010)是AML患者3年死亡的危险因素。结论脾脏肿大、白�
Objective To observe the expressions of serum SALL4 and CASP1 in patients with acute myeloid leukemia(AML),and to investigate their relationships with clinicopathological features and prognosis.Methods Totally 120AML patients were given standardized chemotherapy according to AML subtypes in the First Affiliated Hospital of Zhengzhou University from November 2017 to November 2019(AML group),and another 120 healthy volunteers received physical examination during the same period(control group).The relative expressions of serum SALL4 and CASP1 mRNAs were detected by real-time fluorescence quantitative PCR in two groups,and were compared among AML patients with different clinicopathological features.According to the mean relative expressions of serum CASP1 and SALL4 mRNAs,120 AML patients were divided into 65 patients with relative expression of CASP1 mRNA≥1.01(CASP1 high-expression group)and 55 patients with relative expression of CASP1 mRNA<1.01(CASP1low-expression group),as well as 61 patients with relative expression of SALL4 mRNA≥0.96(SALL4 high-expression group)and 59 patients with relative expression of SALL4 mRNA<0.96(SALL4 low-expression group).The patients were followed up for 3 years to compare the 3-year overall survival rate between CASP1 high-and low-expression groups and between SALL4 high-and low-expression groups.Multivariate Cox regression analysis was performed to assess the risk factors of 3-year mortality of AML patients.Results(1)The relative expressions SALL4 and CASP1 mRNA were higher in AML group(0.96±0.15,1.01±0.22)than those in control group(0.81±0.11,0.78±0.16)(t=6.147,P<0.001;t=6.443,P<0.001).(2)The relative expressions of SALL4 and CASP1 mRNAs were higher in AML patients with splenomegaly and white blood cell count≥10×10^(9)/L(1.02±0.11,1.04±0.11;1.16±0.13,1.12±0.13)than those in AML patients with no splenomegaly and white blood cell count<10×10^(9)/L(0.96±0.08,0.95±0.09;0.91±0.07,0.98±0.09)(P<0.05).There were no significant differences in the relative expressions of S
