摘要
目的 探讨钼靶X射线联合细胞周期素蛋白D1(Cyclin D1)对乳腺癌保乳术切缘状态预测价值,并分析其对手术切缘距离的影响。方法 选取2020-03-01-2021-12-01舞钢市人民医院收治的82例女性乳腺癌保乳术患者为研究对象,根据术中切缘病理及术后病理结果,分为切缘阳性组(n=26)、切缘阴性组(n=56)。比较2组基本临床及病理特征,观察钼靶X射线及钼靶X射线联合Cyclin D1对保乳术切缘状态的检查结果,分析钼靶X射线联合Cyclin D1对保乳术切缘阳性的预测效能及钼靶X射线影像学分类与切缘状态关系。结果 2组Cyclin D1表达、手术切缘距离比较,差异有统计学意义,χ^(2)值分别为24.777和7.280,P值分别为<0.001和0.007。钼靶X射线对保乳术切缘状态预测的敏感度为92.31%、特异度为82.14%、准确度为85.37%、误诊率为17.86%、漏诊率为7.69%、阴性预测值为95.83%、阳性预测值为70.59%。钼靶X射线联合Cyclin D1对保乳术切缘状态的预测敏感度为96.15%,高于钼靶X射线的92.31%、Cyclin D1的76.92%,χ^(2)=7.800,P=0.020。钼靶影像学分类中混合类型的保乳术切缘状态差异有统计学意义,χ^(2)=10.040,P=0.018。结论 钼靶X射线、Cyclin D1联合检查可有效提高对保乳术切缘状态的预测敏感度,改善保乳术预后。
Objective To investigate the predictive value of molybdenum target X-ray combined with Cyclin Dl on the state of resection margin during breast-conserving surgery,and to analyze its effect on the surgical margin distance.Methods Eighty-two female patients with breast cancer who underwent breast-conserving surgery and who were admitted to the Maigang People's Hospital from March 1,2020 to December 1,2021 were selected for the study,and they were divided into a positive margin group(n=26)and a negative margin group(n=56)according to the intraoperative margin pathology and postoperative pathology results.The results of mammography and mammography combined with Cyclin D1 on the status of breast-conserving incision margins were compared,and the predictive efficacy of mammography combined with Cyclin Dl on positive breast-conserving incision margins and the relationship between mammography classification and incision margin status were analyzed.Results There were significant differences in Cyclin Dl expression and surgical margin distance between the two groups,the χ^(2) values were 24.777 and 7.280 respectively,and the P values were<0.001 and 0.007 respectively.The sensitivity,specificity,accuracy,misdiagnosis rate,missed diagnosis rate,negative predictive value and positive predictive value were 92.31%,82.14%,85.37%,17.86%,7.69%,95.83%,70.59%,respectively.The pre diction sensitivity of molybdenum target X-ray combined with Cyclin D1(96.15%)was higher than that of molybdenum target X-ray(92.31%)and Cyclin D1(76.92%),χ^(2)=7.800,P=0.020.Molybdenum imaging classification of mixed types of breast conservation surgery had significant differences in cutting edge status,χ^(2)=10.040,P=0.018.Conclusion Molybdenum target X-ray and Cyclin D1 combined examination can effectively improve the sensitivity of the prediction of cutting edge status and improve the prognosis of breast-conserving surgery.
