电化学发光法在乙肝表面抗原检测中的应用研究

摘要 目的对电化学发光免疫技术测定乙肝表面抗原(HBsAg)做出方法学评价,并对乙肝病毒携带者中HBsAg进行滴度检测,探讨其与乙肝病毒(HBV)复制的相关性。方法选取2018年10月-2019年12月来天津市第一中心医院体检的HBsAg阳性、肝功能正常、肝脏超声无异常体检者152例为研究对象。对152例乙肝携带者血清进行HBsAg滴度、乙肝e抗原(HBeAg)及乙肝病毒核酸(HBV-DNA)检测。HBeAg采用酶联免疫吸附实验(ELISA)检测,HBV-DNA载量采用聚合酶链反应(PCR)检测,HBsAg滴度采用电化学发光法半定量检测。将病例按照HBV-DNA水平分为4组,HBV-DNA水平分别为<10^(3)、10^(3)~10^(5)、>10^(5)~10^(7)及>10^(7)copies/mL。结果HBsAg滴度与HBV-DNA水平呈显著对数相关(r=0.630,P<0.01),根据ROC曲线,Ⅳ组鉴别于其他各组的最佳临界值cut-off为HBsAg滴度(COI)=9250,而Ⅰ组鉴别于其他各组的最佳cut-off为HBsAg滴度(COI)=1250。结论HBsAg滴度检测方法经济、简便,可替代PCR方法判断HBV复制水平。在没有条件开展PCR技术的实验室,采用该方法检测HBsAg滴度可评价患者体内病毒复制情况,从而指导临床用药或做进一步检查。 Objective To evaluate the methodology of detecting hepatitis B surface antigen(HBsAg)by electrochemiluminescence immunoassay,and to detect the titer of HBsAg in hepatitis B carriers,to explore its correlation with hepatitis B virus(HBV)replication.Methods A total of 152 physical examinees who came to Tianjin First Central Hospital from October 2018 to December 2019 and had positive HBsAg,normal liver function and no abnormal liver ultrasound were selected as the study subjects.HBsAg titer,hepatitis B e antigen(HBeAg)and hepatitis B virus nucleic acid(HBV-DNA)of 152 hepatitis B carriers were detected.HBeAg was detected by enzyme-linked immunosorbent assay(ELISA),HBV-DNA load was detected by polymerase chain reaction(PCR),and HBsAg titer was detected by semi quantitative electrochemiluminescence method.According to the results of HBV-DNA detection,the patients were divided into 4 groups,the HBV-DNA levels were<10^(3),10^(3)-10^(5),>10^(5)-10^(7) and>10^(7) copies/mL,respectively.Results HBsAg titer was significantly logarithmically correlated with HBV-DNA level(r=0.630,P<0.01).According to ROC curve,the optimal cut-off value for HBsAg titer(COI)=9,250 in group Ⅳ was identified in other groups,while the optimal cut-off value for HBsAg titer(COI)=1,250 in group Ⅰ was identified in other groups.Conclusion HBsAg titer detection method was economical and simple,and could replace PCR method to determine the level of HBV replication.In laboratories where PCR technology was not available,using this method to detect HBsAg titers could evaluate the virus replication status in patients,thus guiding clinical medication or further testing.
