累积性轴向载荷及复合运动无创性构建犬腰椎间盘退变动物模型

Establishing a noninvasive canine model of lumbar intervertebral disc degeneration through cumulative axial load and compound motion

摘要 目的探讨应用自主研发的腰椎复合运动机对直立体位犬腰椎施加累积性轴向载荷和六位相复合运动构建犬腰椎间盘退变模型的可行性。方法成年雌性草犬20只,体重(10.0±0.5)kg,随机分为两组,每组10只。模型组:将实验动物以垂直坐位固定于运动机上,在持续轴向载荷下(245 N)进行一定周期的腰椎六位相(前屈后伸、左右侧屈、左右旋转各45°)复合运动;对照组:将实验动物以垂直坐位固定于运动机上,不予任何干预。模型组动物在进行复合运动前、运动2、5、10及15万次时行腰椎影像学检查,侧位X线片测定椎间盘高度指数(disc height index,DHI),MRI T2-mapping成像定量分析椎间盘退变程度,当MRI T2WI显示椎间盘出现明显退变(改良pfirrmann分级系统>V级)时停止复合运动。取两组动物腰椎各节段椎间盘标本,通过Micro-CT定量分析上、下终板骨密度(bone mineral density,BMD)及骨小梁结构,组织学染色(HE染色、番红"O"染色、天狼猩红染色)验证及评价椎间盘退变程度。结果模型组动物5万次复合运动后L_(6-7)、L_(7)S_(1)椎间盘MRI T2WI呈轻度退变,随着运动负荷累积其退变程度逐渐加重;至复合运动10万次时达到中度退变,表现为DHI开始降低,同时L_(5-6)椎间盘也出现轻度退变;当复合运动量15万次时,L_(5-6)、L_(6-7)、L_(7)S_(1)椎间盘高度进一步降低,椎间盘呈严重退变(改良pfirrmann分级系统Ⅳ~Ⅵ级),而上位L 4-5椎间盘也级联出现轻度退变。L_(5-6)椎间盘组织学评分为(8.2±0.8)分、L_(6-7)为(9.5±0.7)分、L_(7)S_(1)为(10.3±0.5)分。退变程度表现为L_(5-6)<L_(6-7)<L_(7)S_(1)。HE及番红"O"染色证实L_(5-6)、L_(6-7)、L_(7)S_(1)椎间盘高度塌陷,髓核组织严重皱缩,内部结构松散、紊乱,几乎未见空泡样髓核细胞;天狼猩红染色示椎间盘前后环纤维均存在明显褶皱、排列紊乱、明显多处断裂,椎间盘后环明显重于前环,且骨性终板厚度� Objective To investigate the feasibility of establishing a canine model of lumbar intervertebral disc degeneration through the application of cumulative axial load and a six-phase combined motion on the vertical sitting dog's lumbar spine.Methods Twenty adult female grass dogs,each weighing 10.0±0.5 kg,were randomly divided into two groups,with 10 dogs in each group.In the model group,dogs were secured to an exercise machine in a vertical position,and six phases of lumbar spine movement(flexion and extension,left and right lateral flexion,left and right rotation,45°each)were combined with a specific number of cycles under continuous axial load(245 N).In the control group,dogs were secured to the exercise machine in a vertical position without any intervention.Radiographic examinations were performed before and after 20,000,50,000,100,000,and 150,000 compound exercises in the model group.The disc height index(DHI)was measured through lateral X-ray,and MRI T2-mapping was used for quantitative analysis of intervertebral disc degeneration.When intervertebral disc degeneration was evident on MRI T2-weighted imaging(modified Pfirrmann system>Grade V),the combined motion was halted.Micro-CT quantitative analysis of bone mineral density(BMD)in the upper and lower endplates,trabecular bone structure,and histological staining(HE staining,"O"staining,Sirius red staining)were employed to verify and assess the degree of intervertebral disc degeneration.Results After 50,000 compound exercises,mild degeneration of the intervertebral discs at L_(6-7)and L_(7)S_(1)was observed on T2-weighted imaging.With the accumulation of exercise load,the degree of degeneration progressively increased,reaching a moderate degree of degeneration after 100,000 composite exercises,and DHI began to decrease.Mild degeneration was also observed in the upper L_(5-6)intervertebral disc.When the cumulative exercise volume reached 150,000 repetitions,the height of intervertebral spaces in the L_(5-6),L_(6-7),and L_(7)S_(1)segments further decreased,and
