摘要
目的利用自研定制的腰椎复合运动机使犬腰椎模拟人类脊柱生物力学及运动学特点,通过累积性施加腰椎运动负荷,无创性构建腰椎椎间盘退行性变动物模型。方法将20只成年雌性草犬随机分为实验组和对照组,每组10只。实验组犬麻醉后以垂直坐位固定于运动机上,在持续轴向载荷下(245 N)进行一定周期数的腰椎六位相(前屈、后伸、左右侧曲、左右旋转各45°)复合运动,每天运动6 h;对照组犬麻醉后以垂直坐位固定于运动机上,不予任何干预。实验组犬在进行复合运动前及运动2万、5万、10万、15万次时进行腰椎影像学检查,采用侧位X线片测定椎间隙高度指数(DHI)下降情况,MRI及T_(2)-mapping成像定量分析评定椎间盘退行性变程度,采用改良Pfirrmann分级系统进行退行性变程度分级,当椎间盘在MRI T_(2)加权像上出现明显退行性变(改良Pfirrmann分级>5级)时即认定为造模成功,停止复合运动,观察是否存在逆转现象。结果实验组犬下腰椎3个节段DHI值随着复合运动量的累积而缓慢下降,当复合运动10万次时,L_(6)/L_(7)、L_(7)/S_(1)的DHI值较5万次时明显下降,差异有统计学意义(P<0.05);至15万次时,DHI值进一步下降,L_(5)/L_(6)、L_(6)/L_(7)、L_(7)/S_(1) DHI值较10万次时明显降低,差异有统计学意义(P<0.05)。实验组犬MRI及T_(2)-mapping值变化类似于DHI值,随着复合运动量的累积,下腰椎L_(5)/L_(6)、L_(6)/L_(7)、L_(7)/S_(1)3个节段退行性变呈逐渐加重趋势;当复合运动5万次时,L_(6)/L_(7)、L_(7)/S_(1)椎间盘在MRI T_(2)加权像上出现轻度退行性变现象(改良Pfirrmann分级2级),其T_(2)-mapping值<2万次时,差异有统计学意义(P<0.05);复合运动10万次时,L_(6)/L_(7)、L_(7)/S_(1)椎间盘退行性变进展为中度(改良Pfirrmann分级3级),且L_(5)/L_(6)椎间盘也出现轻度退行性变,其T_(2)-mapping值<5万次时,差异有统计学意义(P<0.05);当复合运动量累积到15万
Objective To make the dog lumbar spine simulate the biomechanics and kinematics characteristics of human spine by using the self-developed lumbar compound motion machine,and to build a non-invasive animal model of lumbar intervertebral disc degeneration by cumulatively applying the lumbar motor load.Methods Twenty adult female dogs were randomly divided into experimental group and control group(n=10).In the experimental group,the dogs were fixed in a upright sitting position on the machine after anesthesia and subjected to a certain number of periods of compound motion(flexion,extension,left and right lateral flexion,left and right rotation,45°each)under continuous axial load(245 N),for 6h a day.In the control group,the dogs were fixed in a upright sitting position on the machine after anesthesia without any intervention.In the experimental group,lumbar imaging was collected before and after motion for 20000,50000,100000,and 150000 times,including lateral roentgenograph to measure the decline of intervertebral height index(DHI),MRI and T_(2)-mapping to quantitatively analyze the degree of intervertebral disc degeneration.The degeneration was graded with modified Pfirrmann intervertebral disc degeneration grading system.When the disc degeneration on MRI T_(2)-weighted was obvious(modified Pfirrmann grade>5),the model was considered as successful,and the compound motion was stopped to observe whether there was reversal phenomenon.Results The DHI values of the 3 segments of the lower lumbar vertebra in the experimental group decreased slowly with the accumulation of compound motion.When the compound motion was performed for 100000 times,the DHI values of L_(6)/L_(7) and L_(7)/S_(1) decreased significantly compared with those of 50000 times,all with a statistical significance(P<0.05).When the compound motion was 150000 times,the decrease degree was further aggravated,and the DHI values of L_(5)/L_(6),L_(6)/L_(7),L_(7)/S_(1) were significantly reduced compared with those of 100000 times,all with a statistical signifi
