摘要
目的 探讨分析糖类抗原125(CA12-5)、糖类抗原153(CA15-3)以及血清癌胚抗原(CEA)三项联合检测对于乳腺癌诊断的临床意义。方法 选取2021年3月-2023年1月来本院就诊的61例乳腺癌患者和53例乳腺良性肿瘤患者为研究对象,分别设为乳腺癌组和良性组,另取同期50名健康体检人员,设为对照组,分别检测三组的CA12-5、CA15-3及CEA表达水平,分析乳腺癌病例临床分期与血清肿瘤标志物的关系,以术后病理诊断为金标准计算CA15-3、CA12-5、CEA单项检测及三项联合检测乳腺癌的敏感性和特异度。结果对照组、良性组和乳腺癌组受检者三项血清肿瘤标志物表达水平存在显著差异,组间对比具有统计学意义(P<0.05);与对照组相比,良性组和乳腺癌组受检者三项血清肿瘤标志物表达水平明显升高,组间对比具有统计学意义(P<0.05),其中乳腺癌组患者三项血清肿瘤标志物表达水平较良性组更高,组间对比具有统计学意义(P<0.05)。与I、Ⅱ期乳腺癌病例相比,Ⅲ、Ⅳ期患者三项血清肿瘤标志物表达水平明显更高,组间对比具有统计学意义(P<0.05);CA12-5+CA15-3+CEA联合检测诊断敏感度更高,与三项标志物单独检测相比差异显著(P<0.05)。结论 在乳腺癌的临床诊断中,CA12-5、CA15-3以及CEA是三项重要的血清标志物指标,对乳腺癌的早期检测有指导意义,三者联合检测弥补了单一指标检测的缺陷和不足,能够提高诊断准确性,对临床鉴别诊断病灶良恶性、评估疾病严重程度有重要的参考价值。
Objective To explore clinical significance of combined detection of carbohydrate antigen 125(CA12-5),carbohydrate antigen 153(CA15-3)and serum carcinoembryonic antigen(CEA)in diagnosis of breast cancer.Methods The paper chose 61 patients with breast cancer in our hospital from March 2021 to January 2023 as research subject,and chose 53 patients with benign breast tumor and 50 healthy people treated with physical examination at the same time as control.Expression levels of CA12-5 and CA15-3 CEA among three groups were detected respectively.Relationship between clinical stages of breast cancer cases and serum tumor markers was analyzed.Sensitivity and specificity of CA15-3,CA12-5,CEA single test and combined test was caculated for breast cancer with gold standard of postoperative pathological diagnosis.Results There was significant difference in expression levels of three serum tumor markers among healthy control group,benign group and breast cancer group,comparison between groups was statistically significant(P<0.05).Expression levels of three serum tumor markers in benign group and breast cancer group were significantly higher than healthy control group,comparison between the groups was statistically significant(P<0.05).Expression levels of three serum tumor markers in breast cancer group were higher than benign group,comparison between the groups was statistically significant(P<0.05).Expression levels of three serum tumor markers in stageⅢandⅣpatients were significantly higher than stage I andⅡ,with statistical significance between the groups(P<0.05).Diagnostic sensitivity of CA12-5+CA15-3+CEA combined detection was higher,with more significant difference than single detection of three biomarkers(P<0.05).Conclusion In clinical diagnosis of breast cancer,CA12-5,CA15-3 and CEA are three important serum markers,which have guiding significance for early detection of breast cancer.Combined detection of three indicators can improve defect of single indicator detection,make up for deficiency of single indicator d
