摘要
目的初步了解引起脑膜炎症状单核细胞增生李斯特菌的流行特征、PFGE型别、血清型、耐药性与临床症状及预后的关联性,并与本地区食品分离株进行比对分析,为该病防治提供参考基础。方法对25株分离自脑膜炎病例中的单核细胞增生李斯特菌采用肉汤稀释法、血清凝集和脉冲场凝胶电泳(PFGE)进行药敏、血清及分子分型研究。结果25株单核细胞增生李斯特菌共分为4个血清型,分别为1/2a、1/2b、3b、4b,流行优势血清型为4b(12株,占48%);所有菌株均对13种抗生素产生较高的耐药性,其中23株多重耐药菌株具有8种耐药谱,优势耐药谱为VAN-ETE-MER-GEN-IPM。25株菌经AscI酶切后,共分为22个不同的PFGE带型,相似度为65.4%~96.0%,PFGE型别呈现较高的多样性,与血清型及预后具有一定的对应性,与耐药性及临床症状没有明显的关联性,与食品来源菌株具有较高的同源性。结论单核细胞增生李斯特菌在宁夏引起脑膜炎呈散发分布,高致病性血清型菌株流行及高耐药性需要引起高度关注。
Objective To understand the epidemiological characteristics,drug resistance and the correlation between PFGE type,serotype,drug resistance,clinical symptoms and prognosis of meningitis symptoms caused by Listeria monocytogenes,and to provide a reference basis for the prevention and treatment of this disease.Methods Serum agglutination and pulsed field gel electrophoresis(PFGE)were used to study drug susceptibility,serum and molecular typing of 25 strains of Listeria monocytogenes isolated from meningitis cases.Results The 25 strains of Listeria monocytogenes were divided into four serotypes(1/2a,1/2b,3b and 4b).The dominant serotype of Listeria monocytogenes isolates was 4b(12/25,48%).All the strains showed high resistance to 14 antibiotics,among which 23 strains showed multiple resistance with different degrees.A total of eight resistance spectra were found,and the dominant resistance spectrum was VAN-ETE-MER-GEN-IPM.After ASCI digestion,25 strains were divided into 22 different PFGE bands which ranged from 65.4%to 96.0%with similarity and without dominant band type.PFGE types showed high diversity,which corresponded to serotype and prognosis and which had no obvious correlation with drug resistance and clinical symptoms.Conclusion Meningitis caused by Listeria monocytogenes is sporadic in Ningxia,and the prevalence of highly pathogenic serotypes and high drug resistance should be paid great attention to it.
