摘要
目的分析非布司他联合复方α-酮酸片对老年慢性肾功能不全患者的疗效。方法选取2021年4月至2022年4月华中科技大学同济医学院附属梨园医院收治的94例老年慢性肾功能不全患者为研究对象,采用随机数字表法分为复方α-酮酸片组(给予复方α-酮酸片,30例)、非布司他组(给予非布司他,31例)和联合组(给予复方α-酮酸片联合非布司他,33例)。比较分析三组患者治疗前后的肾功能[血肌酐(serum creatinine,Scr)、24h尿蛋白定量、尿素氮(blood urea nitrogen,BUN)]、血脂水平[甘油三酯(triglyceride,TG)、总胆固醇(total cholesterol,TC)]、内皮功能[一氧化氮(nitric oxide,NO)、内皮素-1(endothelin-1,ET-1)]、炎症因子[C反应蛋白(C-reactive protein,CRP)、白介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)]水平。比较分析三组患者治疗结束后的临床疗效以及治疗期间的不良反应发生情况。结果与治疗前比较,三组患者治疗结束后的Scr、24h尿蛋白定量、BUN、TG、TC、ET-1、CRP、IL-6、TNF-α均显著降低(P<0.05);治疗结束后,联合组患者的Scr、24h尿蛋白定量、BUN、TG、TC、ET-1、CRP、IL-6、TNF-α均显著低于复方α-酮酸片组和非布司他组(P<0.05),而复方α-酮酸片组与非布司他组比较差异均无显著性(P>0.05)。与治疗前比较,三组患者治疗结束后的NO显著升高(P<0.05);治疗结束后,联合组患者的NO、总有效率均显著高于复方α-酮酸片组和非布司他组(P<0.05),而复方α-酮酸片组与非布司他组比较差异均无显著性(P>0.05)。三组患者治疗期间的不良反应发生情况比较差异均无显著性(P>0.05)。结论非布司他联合复方α-酮酸片可改善老年慢性肾功能不全患者的肾功能、内皮功能,降低患者的血脂水平,减轻炎症反应,显著提升患者的临床疗效。
Objective To analyze the efficacy of febuxostat combined with compound alpha-ketoic acid tablets in elderly patients with chronic renal insufficiency.Method 94 elderly patients with chronic renal insufficiency admitted to Liyuan Hospital,Tongji Medical College,Huazhong University of Science and Technology from April 2021 to April 2022 were selected as the study objects.They were divided into compound alpha-ketoacid tablet group(given compound alpha-ketoacid tablet,30 cases),feburestat group(given feburestat,31 cases)and combination group(given compound alpha-ketoacid tablets combined with febuxostat,33 cases)by random number table method.Renal function(Scr,24h urinary protein,BUN),lipid levels(TG,TC),endothelial function(NO,ET-1)and inflammatory factors(CRP,IL-6,TNF-α)of the three groups were compared and analyzed before and after treatment.The clinical efficacy after treatment and the occurrence of adverse reactions during treatment of the three groups were compared and analyzed.Result Compared with before treatment,Scr,24h urinary protein,BUN,TG,TC,ET-1,CRP,IL-6 and TNF-αwere significantly decreased in three groups after treatment(P<0.05).After treatment,Scr,24h urinary protein,BUN,TG,TC,ET-1,CRP,IL-6 and TNF-αin combination group were significantly lower than those in compound alpha-ketoacid tablet group and feburestat group(P<0.05),while there was no significant difference between compound alpha-ketoacid tablet group and feburestat group(P>0.05).Compared with before treatment,NO was significantly increased in three groups after treatment(P<0.05).After treatment,the NO and total effective rate in combination group were significantly higher than those in compound alpha-ketoacid tablet group and febuxoat group(P<0.05),while there was no significant difference between compound alpha-ketoacid tablet group and febuxoat group(P>0.05).There was no significant difference in the occurrence of adverse reactions among the three groups during treatment(P>0.05).Conclusion Febuxoat combined with compound alpha-ketoic aci
