摘要
目的 探讨老年糖尿病患者胰岛素注射知信行现状及培训需求,并分析其影响因素。方法 随机选取2019年1—12月在我院内分泌科接受治疗的422例老年糖尿病患者作为研究对象,采用一般情况调查问卷、胰岛素注射知信行问卷及胰岛素注射培训需求调查表进行问卷调查,并对数据进行统计分析。结果研究对象胰岛素注射知信行问卷总得分为(44.06±8.55)分。研究对象胰岛素注射培训需求问卷得分为(23.79±2.79)分,有强烈培训需求的患者(需求问卷得分≥25分)共235人(55.67%),有培训需求的患者(需求问卷得分≥10分)共398人(94.31%),得分较高的需求条目为“胰岛素分类及作用”“胰岛素注射部位的选择”。不同性别、文化水平、病程的研究对象的胰岛素注射行为得分差异均有统计学意义(P<0.05)。而不同社会人口学特征的研究对象的胰岛素注射培训需求问卷得分差异均无统计学意义(P>0.05)。多元线性回归分析结果显示:文化水平和病程是研究对象胰岛素注射行为的影响因素(P<0.05)。结论 研究对象的胰岛素注射知信行水平相对较低,为了更好地提高老年糖尿病患者的胰岛素注射知信行水平,应根据患者的性别、文化水平、病程及患者的培训需求,制定针对性的干预方案。
Objective This paper aims to investigate the current status of Knowledge-Attitud-Practice(KAP)and training needs of insulin injection in elderly patients with diabetes mellitus and to analyze the influencing factors.Methods A total of 422 elderly patients with diabetes mellitus who received treatment in the Department of Endocrinology of our hospital from January to December 2019 were randomly selected as the participants.The general status questionnaire,KAP questionnaire of insulin injection knowledge,and insulin injection training needs questionnaire were used for the questionnaire survey.Data were statistically analyzed.Results The total score of the KAP questionnaire of insulin injection knowledge was(44.06±8.55)points.The scores of the insulin injection training needs questionnaire were(23.79±2.79)points,235(55.67%)patients with strong training needs(needs score≥25 points)and 398(94.31%)patients with training needs(needs score≥10 points).The highest needs category was″insulin classification and action″and″insulin injection site selection″.Significant differences were found in insulin injection behavior scores among participants of different genders,education levels,and duration of disease(P<0.05).However,no significant differences were found in scores of the insulin injection training needs questionnaire among participants with different sociodemographic characteristics(P>0.05).The results of multiple linear regression analysis showed that education level and duration of disease were the influencing factors of insulin injection behavior(P<0.05).Conclusion The level of KAP of insulin injection in elderly patients with diabetes is relatively low.To improve the level of KAP of insulin injection in them,a targeted intervention program should be developed following the gender,education level,duration of disease,and training needs of patients.
