摘要
目的:探讨腹腔镜子宫肌瘤剔除术(Laparoscopic myomectomy,LM)对子宫肌瘤患者内分泌指标和并发症的影响。方法:选取2019年5月至2023年5月本院收治的子宫肌瘤患者70例,根据不同手术方式分组,其中对照组32例采用传统开腹子宫肌瘤剔除术(Traditional myomectomy,TAM),观察组38例采用LM。比较两组患者围术期指标(手术时间、术中出血量、术后首次排气时间、住院时间);比较术前及术后第2d患者内分泌指标(卵泡雌激素、雌二醇、黄体生成素)、氧化损伤指标(缺血修饰清蛋白、肌红蛋白)、免疫功能T淋巴细胞(CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+))及术后并发症总发生率。结果:观察组各项围术期指标优于对照组(P<0.05)。术前比较两组患者各项内分泌指标、氧化损伤指标、免疫功能指标均无明显差异(P>0.05);术后第2d,两组卵泡雌激素、黄体生成素水平明显升高,雌二醇水平明显降低,且观察组卵泡雌激素、黄体生成素水平低于对照组,雌二醇水平高于对照组(P<0.05);两组缺血修饰清蛋白、肌红蛋白水平明显升高,且观察组缺血修饰清蛋白、肌红蛋白水平明显低于对照组(P<0.05);两组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平明显降低,且观察组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平明显高于对照组(P<0.05);观察组并发症总发生率较对照组明显降低(P<0.05)。结论:LM治疗子宫肌瘤患者能缩短手术时间、减少术中出血量,缩短预后时间,并有效缓解氧化损伤,改善患者内分泌,降低并发症发生率。
Objective:To explore the impact of laparoscopic myomectomy(LM)on endocrine indicators and complications in patients with uterine fibroids.Method:70 patients with uterine fibroids admitted to our hospital from May 2019 to May 2023 were selected and divided into a control group(32 cases treated with traditional myomectomy(TAM))and an observation group(38 cases treated with LM)based on different surgical methods.Compare perioperative indicators(surgical time,first postoperative exhaust time,hospital stay,intraoperative bleeding volume)between two groups of patients;Compare preoperative and postoperative endocrine indicators(follicle estrogen(FSH),estradiol(E2),luteinizing hormone(LH)),oxidative damage indicators(ischemic modified albumin(IMA),myoglobin(MYO)),immune function(T lymphocytes(CD3^(+),CD4^(+),CD4^(+)/CD8^(+)))and postoperative complication incidence.Result:The perioperative indicators of the observation group were better than those of the control group(P<0.05);After surgery,the levels of FSH,LH,IMA,and MYO in the observation group were lower than those in the control group(P<0.05),and the E2 levels were higher than those in the control group(P<0.05);The levels of various immune functions in the observation group were higher than those in the control group(P<0.05),and the total incidence of complications was lower than that in the control group(P<0.05).Conclusion:LM treatment for uterine fibroids can shorten surgical time,reduce intraoperative bleeding,shorten prognosis time,effectively alleviate oxidative damage,improve patient endocrine function,and reduce the incidence of complications.
