摘要
目的探讨比较肝脏局灶性病变Sonazoid超声造影(Sonazoid contrast-enhanced ultrasound,SE-CEUS)增强模式及造影定量参数的区别。方法选取2022年1~9月笔者医院诊治的56例肝脏局灶性病变患者,对其行SE-CEUS检查,根据病理或结合增强磁共振及增强CT检查结果分为恶性组和良性组,对比两组间超声造影特征及超声造影定量参数。结果恶性组病灶增强模式以“快进快出”为主,库普弗期均有不同程度的廓清。良性组增强模式以“同进同出”为主,绝大多数病灶库普弗期未见廓清。恶性组较良性组到达峰值的时间点(time to peak,TTP)、上升时间(rise time,RT)均低(P<0.05),两组间开始出现的时间点(arrival time,AT)、峰值强度(perk in tensity,PI)、曲线下面积(area under curve,AUC)差异无统计学意义。恶性组病灶与周边实质比较,AT、TTP较短,PI、AUC较大(P均<0.05)。结论SE-CEUS在肝脏局灶性病变血管相的表现与既往Sonovue超声造影表现一致,特有的库普弗期可以帮助医生做出诊断。对于增强不典型的病灶还可参考超声造影定量参数进行判断。
Objective To compare the differences in Sonazoid contrast-enhanced ultrasound(SE-CEUS)enhancement patterns and quantitative characteristics between different kinds of focal liver lesions.Methods From January 2022 to September 2022,56 patients with focal liver lesions who received treatment at our hospital were selected and subjected to a SE-CEUS examination.They were divided into a benign group and a malignant group based on the findings of pathology or other enhanced imaging examination.Comparisons were made between the two groups'variations in contrast-enhanced ultrasound characteristics and parameters.Results The malignant group's enhancement mode largely consisted of quick enhancement in the early arterial phase,washout in the portal venous phase,and washout to different degrees in the late vascular phase.The degree of enhancement didn't start to decline until the late vascular phase in the majority of benign lesions,which exhibit synchronous enhancement and washout with peripheral liver tissue.There was no statistically significant difference in arrival time(AT),peak intensity(PI),and are under curve(AUC)between the two groups,while time to peak(TTP)and rise time(RT)were lower in the malignant group than that in the benign group(P<0.05).The malignant group had longer AT and TTP,higher PI and AUC,and shorter AT and TTP when compared to peripheral parenchyma(P<0.05).Conclusion The appearance of SE-CEUS in the vascular phase of focal liver lesions is consistent with previous CEUS findings,and the distinctive Kupffer phase can aid in the identification.The quantitative parameters of contrast-enhanced ultrasound can also be used to determine the lesions with atypical enhancement.
