摘要
目的探讨动态血糖仪(CGMS)监测在社区老年糖尿病患者中的应用效果及影响。方法选取2021年6月至2022年7月河北省张家口市4个社区的120例2型糖尿病老年人作为研究对象,随机分为对照组和观察组,每组60例。对照组采用常规血糖监测,观察组采用动态血糖监测,比较2组的血糖控制水平,自我管理能力和生活质量。结果干预前,2组患者的性别、年龄、病程、体重指数(BMI)和血糖控制水平差异无统计学意义(P>0.05);干预后,观察组空腹血糖(FPG)、餐后2 h血糖水平(2hPG)、糖化血红蛋白(HbA1c)、低密度脂蛋白胆固醇(LDL-C)水平和血糖波动水平均显著低于对照组(P<0.05)。干预前,患者自我管理行为量表(SDSCA)评分和生存质量量表(WHOQOL-BREF)评分差异无统计学意义(P>0.05);干预后,观察组SDSCA和WHOQOL-BREF量表评分均显著高于对照组(P<0.05)。结论CGMS监测在社区老年糖尿病患者应用效果良好,可以改善老年糖尿病患者的血糖监测依从性及自我管理能力,稳定血糖水平,改善生活质量。
Objective To explore the application and effect of a continuous glucose monitoring system(CGMS)on elderly patients with type 2 diabetes mellitus(T2DM)in community.Methods A total of 120 elderly T2DM patients in 4 communities of Zhangjiakou City,Hebei Province from June 2021 to July 2022 were recruited.They were randomly divided into control group and observation group,with 60 cases in each group.Routine blood glucose monitoring and CGMS were performed in control group and observation group,respectively.Blood glucose control levels,self-management and quality of life were compared between groups.Results Before intervention,there were no significant differences in the gender,age,course of disease,body mass index(BMI)and blood glucose control between the two groups(P>0.05).After intervention,fasting blood glucose(FPG),2h postprandial blood glucose level(2hPG),hemoglobin A1C(HbA1C),low density lipoprotein cholesterol(LDL-C)and blood glucose fluctuation level in the observation group were significantly lower than those of control group(P<0.05).There were no significant differences in the Summary of Diabetes Self-Care Activities(SDSCA)and World Health Organization Quality of Life-BREF(WHOQOL-BREF)scores before intervention(P>0.05),which,after intervention were significantly higher in the observation group than those of the control group(P<0.05).Conclusion CGMS has a good effect on elderly T2DM patients in the community,which can improve the compliance of blood glucose monitoring and self-management ability,stabilize the blood glucose level,and improve the quality of life.
