摘要
目的探讨超声引导下在关节腔内玻璃酸钠注射基础上增加膝关节周围软组织液压分离治疗膝关节骨性关节炎的临床疗效。方法回顾性分析宁德市闽东医院疼痛科2021年9月至2022年5月收治的50例膝关节骨性关节炎患者临床数据,按照治疗方法不同将患者分为治疗组和对照组,各25例。其中通过超声引导下膝周软组织液压分离加关节腔内玻璃酸钠注射治疗膝关节骨性关节炎为治疗组;玻璃酸钠关节腔内注射治疗为对照组。比较两组治疗前与治疗后7 d、术后1个月、术后3个月的疼痛强度(VAS评分)、骨关节炎指数WOMAC评分的变化,术后3个月的临床治疗效果(Lequesne指数评分)以及两组不良反应发生率。结果与对照组患者相比,治疗后7 d、术后1个月、术后3个月治疗组VAS评分、WOMAC功能评分均较低(均P<0.05)。术后3个月,治疗组患者总有效率为100.00%,临床效果优于对照组(P<0.05),其中治疗组。两组不良反应总发生率差异无统计学意义。结论超声引导下膝周软组织液压分离加关节腔内玻璃酸钠注射治疗膝骨性关节炎临床疗效好,不良反应小。
Objective To explore the comparative difference in therapeutic effects between ultrasound-guided hydraulic separation of soft tissue around the knee joint and intra-articular injection of sodium hyaluronate and simple intra-articular injection of sodium hyaluronate in knee osteoarthritis.Methods Retrospective analysis of 50 cases of osteoarthritis of the knee joint patients admitted to the pain department of Mindong Hospital in Ningde City from September 2021 to May 2022 Retrospective analysis of the pain department of Mindong Hospital in Ningde City admitted as the study object,the patients were divided into the treatment group and the control group according to the different methods of treatment,25 cases each.Among them,the treatment of knee osteoarthritis by ultrasound-guided hydraulic separation of periprosthetic soft tissues plus intra-articular sodium vitrate injection was the treatment group;the treatment of knee osteoarthritis by intra-articular sodium vitrate injection was the control group.25 cases of knee osteoarthritis were treated by ultrasound-guided hydraulic separation of periprosthetic soft tissues plus intra-articular sodium vitrate injection(treatment group);25 cases of knee osteoarthritis by intra-articular sodium vitrate injection(treatment group);25 cases of Osteoarthritis of the knee joint(control group),comparing the changes in the levels of pain intensity(VAS score)and WOMAC function score of the two groups of patients before and 7 days after the treatment,1 month after the operation,and 3 months after the operation,and the clinical therapeutic effect(Lequesne index score)at 3 months after the operation,as well as the comparative analyses of the incidence of adverse reactions in the two groups.Results Compared with the patients in the control group,the VAS scores and WOMAC function scores were lower in the treatment group at 7 days after treatment,1 month after operation and 3 months after operation(P<0.05).At 3 months postoperatively,the clinical therapeutic effect of patients in the tre
