儿童病毒性肺炎CD8^(+)T活化状态、PD-1、Tim-3及细胞因子变化的研究 被引量:2

Study on CD8^(+)T activation status,PD-1,Tim-3,and cytokine changes in children with viral pneumonia

摘要 为检测病毒性肺炎患儿CD8^(+)T细胞的分子标志物和不同细胞因子的表达水平,分析检测指标之间的相关性,判断疾病状态下机体的免疫功能,选取65例病毒性肺炎患儿和49例健康体检儿童,应用FACS检测CD8^(+)CD38^(+)、CD8^(+)CD28^(+)、CD8^(+)PD-1^(+)、CD8^(+)T细胞免疫球蛋白和黏蛋白结构域分子3(T cell immunoglobulin and mucin domain 3,Tim-3)^(+)T细胞占总淋巴细胞百分比,应用流式微球技术(cytometric bead array,CBA)检测IL-2、IL-4、IL-6、IL-10、IL-17A、TNF-α、IFN-γ细胞因子的水平,并分析具有不同表面标志物的CD8^(+)T细胞与细胞因子的相关性。结果显示,与对照组比较,病毒性肺炎组患儿的CD8^(+)CD38^(+)、CD8^(+)CD28^(+)、CD8^(+)PD-1^(+)、CD8^(+)Tim-3^(+)T细胞比例升高(P<0.05),IL-2、IL-4、IL-6、TNF-α、IFN-γ水平升高(P<0.05),IL-10水平降低(P=0.028);CD8^(+)CD38^(+)T细胞比例与IL-2、IL-4、IL-6、IL-17A、IFN-γ水平呈正相关(P<0.05);CD8^(+)CD28^(+)T细胞比例与IL-10、IFN-γ水平呈正相关(P<0.05),与IL-2、IL-4、IL-6水平呈负相关(P<0.05);CD8^(+)PD-1^(+)T细胞比例与IL-4、IL-6、IL-17A、TNF-α、IFN-γ水平呈正相关(P<0.05);CD8^(+)Tim-3^(+)T细胞比例与IL-2、IL-4、IL-6、TNF-α、IFN-γ水平呈正相关(P<0.05)。不同病毒感染组之间存在差异(P<0.05)。该研究提示,病毒性肺炎患儿存在免疫活化与抑制的紊乱状态和细胞因子的分泌异常,综合检测患儿免疫指标可以评估机体的免疫状态。 This research proposes to determine the immune function of the body in the disease state by analyzing the correlation between the expressions of CD8^(+)T cell molecular markers and cytokines in children with viral pneumonia.FACS was used to detect the CD8^(+)CD38^(+),CD8^(+)CD28^(+),CD8^(+)PD-1^(+),CD8^(+)T cell immunoglobulin and mucin domain 3(Tim-3)^(+)T cells percentage in 65 children with viral pneumonia and those in 49 healthy children and cytometric bead array(CBA)was used to measure the levels of IL-2,IL-4,IL-6,IL-10,IL-17A,TNF-α,and IFN-γ.The correlations between CD8^(+)T cells with different surface markers and cytokines were analyzed.The results showed that compared to those in the control group,the proportion of CD8^(+)CD38^(+),CD8^(+)CD28^(+),CD8^(+)PD-1^(+),and CD8^(+)Tim-3^(+)T cells in the patient group was increased(P<0.05).The levels of IL-2,IL-4,IL-6,TNF-α,and IFN-γwere increased(P<0.05),and the level of IL-10 was decreased(P<0.05).The percentage of CD8^(+)CD38^(+)T cells was positively correlated with the levels of IL-2,IL-4,IL-6,IL-17A,and IFN-γ(P<0.05).The percentage of CD8^(+)CD28^(+)T cells was positively correlated with the levels of IL-10 and IFN-γ(P<0.05),and negatively correlated with the levels of IL-2,IL-4,and IL-6(P<0.05).The percentage of CD8^(+)PD-1^(+)T cells was positively correlated with IL-4,IL-6,IL-17A,TNF-α,and IFN-γ(P<0.05).The percentage of CD8^(+)Tim-3^(+)T cells was positively correlated with IL-2,IL-4,IL-6,TNF-α,and IFN-γ(P<0.05).There were differences among different virus infection groups.The research suggests that immune abnormalities are involved in the development of viral pneumonia in children.The immune status of the children can be assessed by the detection of relevant immune indicators.
