摘要
目的探讨miR-152-5p及miR-210-5p在儿童IgA肾病(IgAN)中的表达及其临床价值。方法选取2018年1月至2022年6月由本院收治的109例IgAN患儿(IgAN组)、100例非IgA肾病的肾炎患儿(非IgAN组)和60例体检正常者(对照组)作为研究对象。其中IgAN患儿根据牛津分型评分(MEST)分为MEST≥3分组(41例)和MEST<3分组(68例),并根据尿蛋白量分为尿蛋白≥1.0g/24h组(56例)和尿蛋白<1.0g/24h组(53例)。比较各组的miR-152-5p、miR-210-5p、IgA/C3及半乳糖缺乏型IgA1分子(Gd-IgA1)水平。绘制受试者工作特征(ROC)曲线分析miR-152-5p、miR-210-5p、IgA/C3及Gd-IgA1诊断IgAN的价值。采用Pearson相关分析miR-152-5p、miR-210-5p表达水平与IgA/C3及Gd-IgA1的相关性。结果IgAN组的miR-152-5p、miR-210-5p、IgA/C3及Gd-IgA1水平均明显高于非IgAN组和对照组(均P<0.001);MEST≥3分组的miR-152-5p、miR-210-5p、IgA/C3及Gd-IgA1水平均明显高于MEST<3分组(均P<0.001);尿蛋白≥1.0g/24h组的miR-152-5p、miR-210-5p、IgA/C3及Gd-IgA1水平均明显高于尿蛋白<1.0g/24h组(均P<0.001)。ROC曲线分析结果显示,miR-152-5p、miR-210-5p、IgA/C3及Gd-IgA1四项联合诊断IgAN的曲线下面积高达0.951(95%CI:0.889~0.994),其灵敏度为97.4%,特异度为85.2%。Pearson相关分析显示,IgAN患儿的血清miR-152-5p、miR-210-5p表达水平与IgA/C3及Gd-IgA1均呈正相关(r=0.796、0.875、0.745、0.817,均P<0.001)。结论miR-152-5p及miR-210-5p在IgAN患儿中呈高表达,与肾脏损伤进展有关,其联合IgA/C3及Gd-IgA1检测对儿童IgAN诊断具有很高的应用价值。
Objective To investigate the expression and clinical value of miR-152-5p and miR-210-5p in children with IgA nephropathy(IgAN).Methods FFrom January 2018 to June 2022,109 children with IgAN(IgAN group),100 children with nephritis without IgA nephropathy(non-IgAN group)and 60 patients with normal physical examination(control group)were selected as the study objects.According to Oxford typing score(MEST),IgAN children were divided into MEST≥3 group(41 cases)and MEST<3 group(68 cases),and urinary protein volume(56 cases)was≥1.0 g/24 h group(56 cases)and urinary protein<1.0 g/24 h group(53 cases).The levels of miR-152-5p,miR-210-5p,IgA/C3 and galactose-deficient IgA1 molecules(Gd-IgA1)were compared among all groups.Receiver operating characteristic(ROC)curve was plotted to analyze the diagnostic value of miR-152-5p,miR-210-5p,IgA/C3 and Gd-IgA1 in IgAN.Pearson correlation was used to analyze the correlation between the expression levels of miR-152-5p,miR-210-5p and IgA/C3 and Gd-IgA1.Results The levels of miR-152-5p,miR-210-5p,IgA/C3 and Gd-IgA1 in IgAN group were significantly higher than those in non-IGAN group and control group(all P<0.001).The levels of miR-152-5p,miR-210-5p,IgA/C3 and Gd-IgA1 in MEST≥3 group were significantly higher than those in MEST<3 group(all P<0.001).The levels of miR-152-5p,miR-210-5p,IgA/C3 and Gd-IgA1 in urine protein≥1.0 g/24 h group were significantly higher than those in urine protein<1.0 g/24 h group(all P<0.001).ROC curve analysis results showed that the area under the curve of the combined diagnosis of IgAN by miR-152-5p,miR-210-5p,IgA/C3 and Gd-IgA1 was as high as 0.951(95%Cl:0.889-0.994),and its sensitivity was 97.4%and specificity was 85.2%.Pearson correlation analysis showed that the expression levels of miR-152-5p and miR-210-5p in serum of IgAN children were positively correlated with IgA/C3 and Gd-IgA1(r=0.796,0.875,0.745,0.817,all P<0.001).Conclusions The miR-152-5p and miR-210-5p are highly expressed in children with IgAN,which is related to the progression of ren
