摘要
目的:通过探讨妊娠哺乳期乳腺囊实性病变的超声征象,同时对比分析超声、钼靶单独检查及二者联合检查对妊娠哺乳期乳腺囊实性病变的诊断效能,提高对于妊娠哺乳期乳腺囊实性病变的诊断水平。方法:回顾性分析2016年1月—2023年3月在我院行超声检查并经病理证实的41例患者共计43个妊娠哺乳期乳腺囊实性病变的超声声像图;其中19例患者共19个病变同期行钼靶检查,对比分析超声、钼靶单独检查及二者联合检查的诊断效能。结果:妊娠哺乳期乳腺囊实性恶性病变超声征象多表现为边缘不光整、有钙化、阻力指数(RI)≥0.7、腋窝淋巴结肿大,与良性病变相比差异有统计学意义(P均<0.05)。超声、钼靶单独检查及二者联合检查诊断妊娠哺乳期囊实性病变的AUC值分别为0.845、0.774、0.857。相比于超声或钼靶单独检查,超声联合钼靶具有更高的灵敏度。结论:边缘不光整、有钙化、腋窝淋巴结肿大RI≥0.7的超声征象对于妊娠哺乳期乳腺囊实性病变的良恶性鉴别具有较高的价值;超声与钼靶联合使用诊断价值更高。
Objective:By exploring the ultrasound signs of complex cystic breast masses during pregnancy and lactation,and then compared the diagnostic value of ultrasound and mammography alone and the combination of both for complex cystic breast masses during pregnancy and lactation to improve the diagnosis of complex cystic breast masses during pregnancy and lactation.Methods:Retrospective analysis was performed for the ultrasound images of 43 cases of complex cystic breast masses during pregnancy and lactation in 41 patients who underwent ultrasound examination and were pathologically confirmed in our hospital from January 2016 to March 2023.Among them,a total of 19 patients(19 lesions)underwent mammography examination at the same time,and the diagnostic value of ultrasound and mammography alone and the combination of both was compared and analyzed.Results:The ultrasound signs of malignant complex cystic breast masses during pregnancy and lactation were mostly characterized by uncircumscribed margins,calcification,resistive index(RI)≥0.7,and enlarged axillary lymph nodes,with statistically significant differences compared to benign lesions(all P<0.05).The AUC values for the diagnosis of complex cystic breast masses during pregnancy and lactation by ultrasound and mammography alone and in combination were 0.845,0.774 and 0.857,respectively.Compared to ultrasound or mammography alone,ultrasound combined with mammography has better sensitivity.Conclusions:Ultrasound signs of uncircumscribed margins,calcification,enlarged axillary lymph nodes,and RI≥0.7 are of high value in the differentiation of benign and malignant complex cystic breast masses during pregnancy and lactation.The combination of ultrasound and mammography has a higher diagnostic value.
